- The VA Pittsburgh Healthcare System (VAPHS), University Drive, Pittsburgh, PA 15240, has a requirement for a non-invasive, diagnosis aiding monitoring system that records all relevant hemodynamic parameters, including heart rate, stroke volume, cardiac output, blood pressure, total peripheral resistance (TPRI), baroreceptor reflex sensitivity (BRRS) and vagal and sympathetic tone. These parameters must be recorded in real time and displayed both as signals and numeric values using the following methods: Continuous (beat-to-beat) blood pressure measurement Impedance cardiography (for SV and CO measurement) High resolution 6-channel-ECG display Oscillometric blood pressure measurement This enables not only an assessment of the cardiovascular system, but also an evaluation of its regulation through the brain. Thus, important functions of the autonomic nervous system - sympathetic and vagal tone - can be determined. This can be used for the assessment of syncope as well as for adjusting pacemakers or for cardiovascular risk stratification. Other fields of application are the management of hypertension, assessment of diabetic neuropathies, or the evaluation of new drugs and vasoactive substances. Requirements: ?Non-invasive and online measurement of all important hemodynamic parameters with no discomfort for the patient ?High resolution 6-channel ECG display (sampling rate 1000 Hz) to guarantee an exact R-R detection. ?Non-invasive beat-to-beat measurement of the blood pressure on the finger with the ?Vascular Unloading Technique?. The absolute values of blood pressure are recorded on a large artery (a. brachialis) with an oscillometric blood pressure device. These values are used to continuously calibrate the beat-to-beat blood pressure values. The derived beat-to-beat values correlate closely with true intra-arterial blood pressure. ?Non-invasive and online measurement of stroke volume, cardiac output and total peripheral resistance with impedance cardiography ? continuously for every heart beat. ?Online recording of heart rate variability and blood pressure variability (HRV, BPV) for an evaluation of the autonomic nervous system and present the results in a 2 or 3-dimensional spectra graphic. ?Online recording of baroreceptor reflex sensitivity. ?Possibility of intervention marks, statistical evaluation of each measurement ?Automatic calculation of trends of hemodynamic parameters, ?sliding power spectra? for the assessment of heart rate and blood pressure variability and of baroreceptor reflex sensitivity (sequence method). ?Saving of the raw data in the standardized "File Exchange Format" (FEF). The file can be loaded at any time in the future for a retrospective off-line analysis of the recorded session ?Facility to export all data into a MS-Excel? file or text file ?High reproducibility for an optimized observation of the patient after therapeutic interventions ?2 additional analogue channels: possible recording of external biosignals.
