- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as Request for Quotation (RFQ) No. FA7012-06-T-0044. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-12. This is set-aside for 100% Small Business. The North American Industry Classification System code is 713940 at $6.5 million size standard. LINE ITEMS: CLIN 0001: To provide a full membership plan including all standard commercial services offered by the fitness facillity, unlimited access during normal operating hours and any and all discounts normally extended to most favored members for MEMBERS O-3 AND BELOW in accordance with the Statement of Work attached hereto. ESTIMATED QTY: 12 (MEMBERS) x UNIT PRICE: $ _____________ (Membership Cost) x 12 Months (1 Yr), EXTENDED PRICE: $ ______________ (NTE). LINE ITEMS: CLIN 0002: To provide a full membership plan including all standard commercial services offered by the fitness facilty, unlimited access during normal operating hours and any and all discounts normally extended to most favored members for MEMBERS O-4 AND ABOVE in accordance with the Statement of Work attached hereto. ESTIMATED QTY: 18 (MEMBERS) x UNIT PRICE: $ _____________ (Membership Cost) x 12 Months (1 Yr), EXTENDED PRICE: $ ______________ (NTE). MEMBERS O-4 AND ABOVE WILL BE CHARGED A CO-PAY OF $18/MONTH. PRICE SHALL REFLECT THE DIFFERENCE BETWEEN THE MEMBERSHIP PRICE AND THE MEMBER?S CO-PAYMENT. ?Notice to Offerors: Funds are not presently available for this project. No award will be made under this combined synopsis/solicitation until funds are available. The Government reserves the right to cancel this solicitation, either before or after the closing date. INSPECTON AND ACCEPTANCE TERMS: Supplies/services will be inspected by Quality Assurance personnel and accepted at job site. DESCRIPTION/SOW: GENERAL INFORMATION. STATEMENT OF WORK FOR AIR FORCE NATIONAL CAPITOL REGION FITNESS REQUIREMENT SILVER SPRINGS. 1. DESCRIPTION OF SERVICES. 1.1. Scope of Work. The contractor shall provide membership benefits to Air Force organizations eligible for coverage under this formal contract. Eligible participants are defined as all active duty Air Force and civilian personnel who are assigned to organizations that are funded through Operating Agency Code (OAC) 15 and work outside of the Pentagon in the Silver Springs Maryland area. 1.2. Requirements. In order to ensure the strength and stamina of Air Force (AF) personnel to complete their respective missions successfully, the Department of Defense provides necessary physical fitness and training areas and facilities. Maintaining the physical fitness of AF personnel is essential to keeping a healthy force ready to deploy in defense of the United States of America. 1.3. OPERATING STANDARDS. AF personnel displaced by the Pentagon renovation or assigned to those organizations without available fitness facilities must have access to fitness facilities and training areas meeting minimum Air Force standards in accordance with AFI 65-106. In accordance with AFM 34-137, Air Force Fitness and Sports Operations, and other Services Directives, fitness facilities contribute to Air Force readiness by enhancing the physical and mental stamina of military and civilian personnel. This is accomplished by providing a variety of programs that contribute to fitness and esprit de corps. 1.4. Physical Fitness Facility Service. In order to support the Air Force mission effectively, the contractor fitness staff must be knowledgeable of preventative health maintenance and weight management. The contractor facility must offer core fitness programs that contribute to cardiovascular fitness, strength conditioning and flexibility, i.e. total fitness. As a minimum the following fitness programs/equipment must be offered: cardiovascular equipment, strength conditioning equipment, aerobic classes and walking and jogging programs. 1.5. Invoice. At the time of submission, members O-4 and above shall be responsible for making a co-payment of $18 per month for each full calendar month remaining in the fiscal year in a lump sum or designated by the contractor. The fitness facility shall submit an invoice to the Program Manager to cover the difference between the total price and the member?s co-payment plus the total cost of all members 0-3 and below who are not required to make a co-payment. Invoices shall be submitted monthly by the fitness facility. The Government shall not be responsible for making a co-payment until the Program Manager receives a completed invoice. If the member elects to initiate membership prior to the first of the next month, then the member shall be entirely responsible for any fee charged (without a Government co-payment). The fitness facility shall submit invoices covering the Government?s portion of the payments to the Air Force Program Manager. Invoices shall be submitted on a monthly basis to: Air Force-National Capital Region Fitness Program Manager, Attn: Anthony Alcala, 130 McChord Street P-15, Bolling AFB, DC 20032. 1.6. Membership. Membership cards issued by the contractor shall identify the name of the government participant along with his or her unit. Any replacement fee for lost cards is the responsibility of the participant. Memberships shall run from the first day of the month following the member?s submission to the fitness facility through the end of the fiscal year (30 September). The rights to guest passes and membership privileges shall belong to the United States Air Force as represented by the Program Manager, and not to the individual participants. 1.6.1. Membership in the fitness facility shall include the following: 1.6.1.1. All standard commercial services offered by the fitness facility. 1.6.1.2. Unlimited access during normal operating hours. 1.6.1.3. Additional discounts normally extended to members. 1.6.2. Not included in Membership. The use of health spas, massages, racquet clubs, golf courses, bowling centers, sporting clubs, and swimming facilities unless included in a standard commercial membership. Benefits to family members of the military/civilian personnel covered by this contract, unless provided at no cost to the government. 1.7. Membership Termination. Participant memberships may be terminated upon 30 days notice with termination effective at the end of the following full-calendar month. Participants who move back to the Pentagon, separate from the Air Force, or are subject to a Permanent Change of Station (PCS) or Permanent Change of Assignment (PCA), must provide supporting documentation to the fitness facility in order to terminate their membership prior to the end of the fiscal year. Any advance payments due the member shall be refunded within 30 calendar days. Additional funds due the Government resulting from membership termination shall be credited on the following month?s invoice. FAR PROVISIONS: The provisions at 52.212-1, Instructions to Offerors-Commercial Items and 52.212-3, Offerors Representation and Certifications-Commercial Items are hereby incorporated. Addendum to FAR Clause 52.212-1 applies to as follows: 52.237-1, 52.252-1, 52.252-5: (b) DFARS (Chapter 2). Offeror must include, with its offer, a completed copy of provision FAR 52.212-3 in full text obtained from http://farsite.hill.af.mil or from http://orca.bpn.gov if registered in ORCA (online Representation and Certifications application). Provision 52.212-2, Evaluation-Commercial Items applies to this solicitation, specifically the following criteria: The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforms to the solicitation and price is determined most advantageous to the Government. FAR CLAUSES: The clause at 52.212-4, Contract Terms and Conditions-Commercial Items is hereby incorporated. Addendum to FAR Clause 52.212-4 applies to this acquisition, specifically the following cited clauses: 52.204-9, 52.209-6, 52.219-6, 52.223-6, 52.224-1, 52.224-2, 52.237-2, 52.252-6: (b) DFARS (Chapter 2), 52.253-1, 252.204-7003, 252.204-7004, 252.212-7001 and 252.232-7003, 252.232-7010, 252.243-7001 and 5352.223-9001. FAR Clause 52.212-5, Contract Terms and Conditions Required to Implement Status of Executive Orders-Commercial Items apply to this acquisition, specifically, the following cited clauses: 52.219-6, 52.222-3, 52.222-21, 52.222-26, 52.222-35, and 52.222.36, 52.222-37, 52.225-3, 52.232-33, 52.233-3 and 52.233-4. All responses must be received, via mail, no later than 4:00 P.M.
