- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented, with the additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. For solicitation number RFQ 502-42-06, award will be made using the simplified acquisition procedures part 13.5. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-12. This acquisition is a small business set-aside. The North American Industry Classification System # 423450 and the small business are 100 employees. The acquisition is for the purchase of 3 each of Audioscan Model Verifit Hearing Aid Analyzer/Real Ear, 3 each user manuals and 1 each part/services manual (brand name), equal or better. If or equal, supply adequate information to determine whether the equal can be used. The quoter shall provide pricing and warranty information on the above listed equipment/supplies. Delivery is to be made to Chief, A&MMS, VA Medical Center, 2495 Shreveport Highway, Pineville, LA 71360. Delivery of equipment is desired between September 25, 2006 and October 13, 2006. Inspection and acceptance is at the VA Medical Center Alexandria, LA. Quote shall specify FOB, VA Medical Center Alexandria, LA (Destination). The provisions at FAR 52.212-1, Instructions of Offeror Commercial Items (Jan 2006) applies to this acquisition. There is an addenda to these provisions. Add the following provisions: FAR 52.252-1, Solicitation Provisions Incorporated by Reference (Feb 1998), This solicitation incorporates one or more solicitation provisions by reference, with the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available. The offeror is cautioned that the listed provision may include blocks that must be completed by the offeror and submitted with its quotation or offer. In lieu of submitting the full text of those provisions, the offeror may identify the provision by paragraph identifier and provide the appropriate information with its quotation or offer. Additional contract requirements: The vendor responding to this acquisition shall provide this office with the following information: a cost per item on company letterhead (equipment and consumables supplies), stating unit price, warranty, FOB, prompt payment terms, correct remittance address, and a completed copy of FAR 52.212-3 (Jun 2006), Offeror Representations and Certifications Commercial Items. If offering an equal product, the vendor must submit documentation of the product with the quote. A company not in possession of FAR 52.212-3 dated Jun 2006 it has been provided as an attachment. Quotes must be in writing and accompanied by a completed Representations and Certifications (FAR 52.212-3) dated Jun 2006 or they will be rejected. Facsimile quotes will be accepted; oral quotes will not be accepted. Award will be made to the offeror whose quote conforms to the requirements of this solicitation, best meets the minimum needs of the Government and has the lowest price, all factors being considered. Award made be made without discussions.
