- The VA Pittsburgh Healthcare System (VAPHS) intends to negotiate on a sole source basis with University of Miami Miller School of Medicine, P.O. Box 016960 (D-41), Miami, Florida 33101 for: 1)cardiopulmonary patient simulator with disease simulations such as blood pressure, venous and arterial pulses, precordial movements, auscultation with appropriate respiratory variation and pulmonary finds for each condition. Includes instructional materials such as PowerPoint slides for curriculum including presentations and discussions of history, bedside findings, ECGs, X-rays, echocardiograms, hemodynamics, angiograms, medical and surgical therapy, pathology, epidemiology and case summary. Also includes teaching manuals, instructors? guide and disease chart. 2) Teaching programs-Series one includes: the normal patient, aortic stenosis, aortic regurgitation, mitral stenosis, mitral regurgitation. Program includes site license, testing program for series, and database program for testing, network installation program. Series two includes: Angina pectoris, acute inferior infarction, acute anterior infarction, hypertension, and cardiomyopathy. Program includes site license, testing program for series, and database program for testing, network installation program. Series three includes: atrial septal defect, acute pericarditis, primary pulmonary hypertension, acute mitral regurgitation, hypertrophic obstructive cardiomyopathy. Program includes site license, testing program for series, and database program for testing, network installation program. 3) Essential Cardiac Auscultation ? site license for unlimited number of learners. Includes learning, practice and self-teaching sections. Covers cardiac ausculatatory findings: S2 splitting, third sound, fourth sound, systolic clicks, innocent murmur, mitral regurgitation, aortic stenosis, aortic regurgitation, mitral stenosis, continuous murmur, tricuspid regurgitation, pericardial rub. The University of Miami Miller School of Medicine provides the only cardiopulmonary patient simulator which has the following features: 1) Designed as a teaching mannequin for the purpose of providing simulation of the clinical and bed side presentation of a minimum of 20-30 cardiac disease states. These disease states should include at a minimum the following simulations: The normal?cardiopulmonary physical examination, an innocent cardiac murmur, a systolic ejection murmur associated with aortic sclerosis, physical findings associated with hypertension, physical findings associated with angina pectoris, physical findings associated with acute myocardial infarction, ventricular aneurysm, mitral valve prolapse, mitral valve click and late systolic murmur, acute and chronic mitral insufficiency, acute and chronic aortic insufficiency, mitral stenosis with tricuspid insufficiency as a result of pulmonary hypertension, aortic stenosis, hypertrophic cardiomyopathy, idiopathic cardiomyopathy, acute pericarditis, pulmonary hypertension, atial septal defect, ventricular septal defect, patent ductus arteriosis,?pulmonary stenosis,? coarctation of the aorta,? tetrology of fallot. ?The mannequin should be based on state of the art digital technology, with?the ability of the simulation to be incorporated into the institutional computer network.?? ?2) The features of the simulated disease presentations begin with summary of the presenting symptoms, available by simulated patient interrogation. The mannequin has the ability to talk. 3) The mannequin is anatomically correct with the following features available for physical examination in a simulated manner - Simulated arterial pulses in the following locations:? bilateral carotid pulses, bilateral femoral pulses, a right brachial pulse, the ability to obtain a blood pressure with simulated Korotkoff sounds. 4) Right and left ventricular precordial impulses. 5) Auscultation in at least 6-9 locations including the aortic, pulmonic, tricuspid, and mitral area. 6) Simulated breath sounds should are available in multiple auscultatory sites. In addition there is a synchronization of the pulses with the heart sounds and an ability to modify the intensity and amplitude of all physical findings. A comprehensive curriculum accompanies the simulator. This consists of instruction manuals, a learner's guide, a self-assessment program with audiovisual displays; and a multimedia computer based cardiology curriculum. Specifically, the multimedia presentation consists of representative results of diagnostic studies for each of the cardiac disease states including electrocardiograms, chest x-rays, echocardiography studies, cardiac catheterization results and a summary of the available therapeutic interventions including operative procedures when appropriate. The simulator is recognized and utilized by national professional organizations such as the American college of cardiology, the British Heart Foundation and the American Board of Internal Medicine. ??????? ??????? ? ??? ? ?
