Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The intent is to award this requirement as a sole source procurement due to the limited number of fitness facilities within walking distance (one mile) of DFAS Arlington. All responsible sources may submit an offer/quote which shall be considered. The solicitation number is HQ0423-06-T-0021. The solicitation is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular, FAC 2001-26, Effective 20 Dec 04. This requirement is UNRESTRICTED. The NAICS code is 713940 and the small business size standard is $6.5 million. The contract line item number(s), quantities and units of measure is as follows: CLIN ITEM QTY UNIT OF ISSUE PERIOD OF PERF BASE PERIOD 0001 Fitness Membership-Military ( 5) 12 Months Oct 1, 2006-Sep 30, 2007 0002 Fitness Membership-Civilians (50) 12 Months Oct 1, 2006-Sep 30, 2007 FIRST OPTION 0003 Fitness Membership-Military ( 5) 12 Months Oct 1, 2007-Sep 30, 2008 0004 Fitness Membership-Civilians (50) 12 Months Oct 1, 2007-Sep 30, 2008 SECOND OPTION 0003 Fitness Membership-Military ( 5) 12 Months Oct 1, 2008-Sep 30, 2009 0004 Fitness Membership-Civilians (50) 12 Months Oct 1, 2008-Sep 30, 2009 Description of requirement for the items to be acquired: STATEMENT OF WORK REQUIREMENT: The contractor shall provide full service physical fitness facilities for civilian and military personnel assigned to the Defense Finance and Accounting Services Headquarters, Arlington, VA SCOPE: The government anticipates purchasing: 5 - Memberships for DFAS Arlington Military Personnel 50 - Memberships for DFAS Arlington Civilian Personnel. Employees shall pay a portion of the membership fee directly to contractor. SPECIFICATIONS: The contractor shall provide: Unlimited access to physical fitness facilities, qualified instructors, formal health programs and services. The facility shall be easily accessible by employees before and after duty hours and during the normal business day. The facility shall be within reasonable walking distance (within one mile) of Crystal Mall 3, 1851 South Bell Street, Arlington, VA. The Contractor shall provide a variety of exercise equipment and classes to include aerobic exercise machines, weight room, pool, aerobic classes, tennis courts, racquetball courts, and shower facilities. The Contractor shall provide numbered, individual identification cards to members at no additional cost to the government. A listing of all cards issued each month and must also maintain a daily sign-in log and prepare reports (i.e. weekly maintenance log/report, monthly fitness center reports, etc.) shall be provided to the point of contact. The Contractor shall ensure that the utilization of memberships does not exceed the estimated number listed under scope, without prior approval from the Contracting Officer. This membership shall belong to the government (DFAS). The Contractor shall propose a mechanism for structuring and pricing a corporate membership for DFAS. In the event that a member transfers or decides not to continue using the facility during the period of this contract the Government (DFAS) shall have the right to transfer the membership from person to person at will, provide that the individual assuming the membership makes the requisite co-payment. There shall be no fee associated with such a transfer. The Government (DFAS) will be allowed to hold PRT testing twice per year at no cost to DFAS military personnel who are not members of the facility. The PRT testing time/date must be approved prior to test. APPLICABLE REGULATIONS AND DOCUMENTS: The following references pertain to the physical fitness requirements and guidelines: 1. DoD Directive 1010.10. 2. Health Promotion and the American College of Sports Medicine Guidelines (ACSM) for Exercise Testing and Prescription. 3. AR 215-1, Morale, Welfare and Recreation Activities. 4. AR 600-63, Army Health Promotion. LENGTH OF CONTRACT: The contract will be for one base period which is anticipated to be from October 1, 2006 through September 30, 2007. Two option years will be included that could be exercised at the discretion of the Government. The proposed procurement contains an option to extend the contract period one or more times at the discretion of the Government. The total duration (base plus options years) shall not exceed three (3) years. Place of performance is Arlington, VA. FOB origin. The provision at 52.212-1, Instructions to Offerors?Commercial, applies to this acquisition. Addenda to FAR 52.212-2 Evaluation ?Commercial Items, is applicable. (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. A single award will be made to the responsible offeror whose offer conforms to the solicitation and is most advantageous to the Government. In making the best value determination, the Government will make a comparative assessment of the offerors. All quotes will be evaluated to determine that the offered price(s) reflect a technical understanding of the requirement and those considered unrealistic may be rejected. Past Performance. Please submit information on your experience and/or past performance performing similar tasks. The offeror may provide past performance information, as follows, for consideration: a. A list of contracts, commercial or Government, performed within the past two years, limited to three (3) Government and three (3) commercial contracts for the same or similar service. The offeror shall provide or include a point of contact (POC) address, telephone number, an average dollar amount of the contract per annum, the period of performance and a sample listing of the services provided for under the commercial contracts. For any Government contracts, provide the Government agency, a (POC), telephone number, appropriate contract number, estimated dollar value of the contract, period of performance and a sample listing of the items provided. The Government reserves the right to limit the number of references it decides to contact. b. A listing of any ?problems? or discrepancies experienced within the past year for the contracts or customers reported in reference to paragraph ?a? above. Include a brief description of how these problems were addressed and remedied. Offerors are to complete paragraph (j) of this provision, 52.212-3, Offeror Representations and Certifications-Commercial Items, if the offeror has completed the annual representations and certifications electronically at http://orca.bpn.gov. If an offeror has not completed the annual representations and certifications electronically at the ORCA website, the offeror shall complete only paragraphs (b) through (i) of this provision.
