- The following Statement of Work is included in this solicitation as an attachment: SCOPE OF WORK ------------------------- The Contractor shall perform all work necessary to provide the services and submittals described in this contract, and is responsible for incidental costs associated with the work, including travel, postage, and administrative costs. Electronic materials prepared for IHS must conform to requirements set forth in Section 508 (29 U.S.C. ? 794d) which provides standard for Federal agencies on how to make their electronic and information technology accessible to people with disabilities. A guide to Section 508 standards is available online at www.section508.gov. ------------------------- I Provide a Needs Assessment Report and Contact List Conduct a survey for IHS Head Start Program on a quarterly basis to determine what the greatest challenges are for local Head Start employees in carrying out their responsibilities. Gather information from directors, health services managers and Head Start employees on their primary health issues and concerns. Prior to soliciting information, collaborate with the IHS Program staff for determining specifically what information will be sought from the local programs. Since Indian Head Start programs are scattered throughout the nation, telephone and email are the most cost effective communication methods. Develop a report for the Director of the IHS Head Start Program summarizing the findings. These findings shall be used in determining the subject material of other deliverables under this contract. Manage a contact list, or listserv, that contains names, email addresses and phone numbers of the Head Start directors and health services managers for each of the 154 program offices. Due to the high attrition rate of Head Start directors and program managers, this list shall be updated quarterly. Submittal: Deliver two copies of the Needs Assessment Report and Contact List to the IHS Head Start Director at within 90 days of contract award, and submit updates for both quarterly thereafter. ------------------------- II Plan and Facilitate Cluster Training and Consortium Workshops Cluster Training. Plan and facilitate two cluster training sessions each lasting approximately two days, for the benefit of Head Start program staff. The training objectives shall cover the primary issues affecting IHS Head Start program offices which may include: mental health, oral health, childhood obesity, health and safety, substance abuse, institutionalizing and integrating staff wellness as a human resource service, and personnel management in meeting employee background screening and checks. Cluster training shall be held at a central location based on those attending. Consortium Workshops. The Contractor shall also provide consultants to participate in the Southwest Consortium of Indian Head Start Programs, (held in Albuquerque, NM), National Indian Head Start Directors Association (Washington DC), and the Northwest Indian Head Start Coalition (Seattle or Portland. These consultants shall conduct eight separate workshops in partnership with the regional and national associations. These workshops will be conducted over a two to three day time period. Objectives for these workshops will be targeted health issues and related health topics, chosen in cooperation with the IHS Head Start Director. These conferences are located in Albuquerque, NM. For both the cluster training and consortium workshops, the Contractor shall be responsible for the following: 1. In consultation with the IHS Head Start Director, compose a pre-training plan which will identify qualified staff to participate in the training and objectives for each session 2. Develop the presentations, training materials, and evaluation tool; 3. Provide training materials in hard copy for mailings, and electronic format for distribution via email and posting on the IHS website; 4. Collect evaluations upon completion of each training session; 5. Provide a written analysis of the evaluations and recommendations for improvement, and 6. Provide a participant list. Submittal: Deliver line items 1 and 2 to the IHS Head Start Director 15 days prior to training dates. Deliver items 3-6 to the IHS Head Start Director within 30 days following each training session. The first training session shall be within 45 days of award. The Contractor shall not be responsible for providing the facility to host any training. Audiographic training. Conduct four teleconference training sessions, one per quarter, which address problems and solutions for Head Start Program health issues. Consult the IHS Head Start Director to identify which staff members will participate in the training, specific topics, and the scope of each teleconference. Topics will be chosen based on the greatest needs identified in Tribal Head Start programs. Develop supplemental materials for distribution via email and posting on the IHS website. Provide the necessary networking systems, bridges and phone lines. The programs can be single focused covering only one topic or can be arranged so that multiple topics are offered focusing on specific geographic areas within IHS Service Areas. Each training teleconference will last approximately 90 minutes. The Contractor is responsible for costs associated with setting up a call center capable of handling up to 20 lines simultaneously, the dissemination of announcements, and providing agendas, support materials, meeting minutes, and participant list. Submittal: Deliver a proposed agenda for each autographic training call to the IHS Head Start Director within 7 days prior to training dates. Provide support materials, meeting minutes, and participant list to the Director within 7 days following each autographic training call. ------------------------- III Consulting Services for the IHS Head Start Advisory Group Provide a qualified consultant to participate in quarterly advisory committee meetings in Albuquerque, NM. Contribute to committee discussions and make meaningful recommendations on agenda topics. The consultant shall have expertise in the area of HS Performance Standards, Tribal Head Start issues, early childhood education and health care issues. Advise how to successfully promote, among the tribal grantees, an understanding of children?s health issues, better access to resources, and how to utilize local mental health resources among Indian communities. Contribute information gathered from the needs assessment conducted in Task I of this statement of work. Participate in one follow up conference call per quarter. Submittal: Provide a written narrative which summarizes the meeting to the IHS Director within 30 days following each meeting. ------------------------- IV Health Bulletins and Dissemination Research and write 10 health bulletins, each on a specific topic based on the needs assessment report conducted under Task I of this statement of work. Topics may include staff wellness, employee background checks, childhood communicable diseases, child nutrition, oral health, early prevention, and program environmental safety. Provide each Health Bulletin in hard copy, and electronic format convenient for emailing and posting on the IHS web page. Submittal: Provide the first Health Bulletin within 60 days of award and one per month thereafter. ------------------------- V Consultants Directory The IHS Head Start Program, in carrying out its charges, taps numerous health experts for workgroup activities, cluster trainings, conference workshops, teleconference programs and resource material development. The Contractor shall maintain a directory of a minimum of 20 consultants which shall include, for each consultant, their contact info and a brief description of their area of expertise. Listed consultants may or may not be affiliated with the Contractor. Submittal: Provide the Consultants Directory to the IHS Director within 60 days of award and update quarterly thereafter. ------------------------- VI ROLES AND RESPONSIBILTIES OF PARTICIPATING GOVERNMENT OFFICIALS The Project Officer shall be responsible for monitoring, assessing, evaluating, and reporting on the Contractor?s technical performance. It is extremely important for the Project Officer to establish and maintain a team oriented line of communication with the Contractor.
