Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is a Request for Quotation (RFQ), Note 1 applies. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circu lar 2005-11. The Army Community Service requires administrative support services to assist in various programs and events for military members and their families, 2000 Hours for the period of performance of 2 October 2006 thru 29 September 2007. Services a re required in accordance with the following Performance Work Statement: 1.0 INTRODUCTION. The Government is soliciting for a Contractor to assist the Government in accomplishing the Army Family Action Plan (AFAP) objectives plus assist in implementing oth er Army Community Service (ACS) programs that include, but are not limited to, Family Advocacy (Celebrating Military Children), Military Spouses Day, Armed Forces Team Building (AFTB) Recognition Day, Installation Volunteer Ceremony. 2.0 BACKGROUND. 2.1.T he mission of the ACS programs is to educate, coordinate and deliver services, which promotes self-reliance, resiliency, readiness and stability within military families. These programs have been supported by contracting for an administrative assistant to prepare and submit reports while coordinating these programs with the ACS Director. 2.2. Armed Forces Action Plan (AFAP). This program identifies and addresses concerns of Americas Military families, determines actions required, and assigns agencies that are located within the Pentagon and Department of the Army to implement actions to resolve specific issues. The AFAP is a grassroots process identifying issues of concern to Americas Armed Forces Family that effect the quality of life for the military. 3 .0 SCOPE. Contractor shall provide services that shall coordinate and execute the ACS programs as specified in Section 5.0 Technical Requirements and 7.0 Supporting Information. The contract shall be for a period of one year. Place of work shall be at the Army Community Service (ACS) Office, Building 4260, Ord Military Community. 4.0 APPLICABLE DOCUMENT. Regulation 608-1, Army Community Service Center. 5.0 TECHNICAL REQUIREMENTS. 5.1. Contractor shall be familiar with and comply with Regulation 608-1 entitl ed Army Community Service Center which will be available at the ACS; Building 4260, Ord Military Community, Seaside, California. 5.2.AFAP Focus Groups: 5.2.1 Contractor shall develop, in coordination with and guidance from the ACS Director, the AFAP Focus Groups, which occurs twice a year. 5.2.2 Contractor shall assist in soliciting issues of well-being, and concerns that have been solicited from the community to include youth/teens, military family members, Navy/NPS, Marines, Air Force, Coast Guard, Califo rnia Medical Detachment, ROTC, and DMDC. 5.2.3. Focus Group Issues: 5.2.3.1 Forums where participants have the opportunity to introduce, review, and discuss issues are at, but not limited to, Town Hall Meetings, Commanders Meetings, Parent/Teacher Meetin gs, BOSS Meetings, and Family Readiness Group Meetings. 5.2.3.2 Contractor shall type issues presented at these meetings plus follow-up plans for clarification with the Government agency/organization that is responsible for the issue. This report shall be submitted to the ACS director at least 90 days before each Focus Group meeting. 5.2.3.4 Contractor shall be responsible for all aspects of the AFAP issues, to include, but not limited to, screening initial entries, researching, coordinating with proponen t offices, maintaining Issue Update book, tracking the issues progress, and apprising principal leaders. 5.2.4 Contractor shall assist the ACS in recruiting mil itary personnel and their family members from paragraph 5.2.2 as well as representatives from the National Guard, Reserves, Retirees, and tenant organizations. 5.2.5 Contractor shall assist ACS in the marketing of the AFAP Focus Groups through community ne ws; flyers; posters; housing newsletters; banners; word of mouth; and other creative means. 5.3 FAP Program Volunteers: 5.3.1 Contractor shall design, develop, and implement effective marketing strategies and public information programs to recruit and util ize volunteers into the AFAP program. 5.3.2 Contractor shall maintain all Government required forms for volunteers that support the AFAP process. 5.4 Advisory Group: 5.4.1 Contractor shall establish and maintain, in coordination with and guidance from the ACS Director, a non-governing advisory group that includes, but is not limited to, officer and non-commissioned officer spouses to include senior leadership spouses. 5.4.2 The advisory group collaborates with the ACS Director and Installation Commanders S teering Committee to implement initiatives that support the AFAP program and accreditation standards. 5.4.3 Contractor shall prepare and submit to the ACS director an after action report within 30 days from the conclusion of each meeting. 5.5 AFAP Policy G uidance/Operating Procedures. Contractor shall be responsible for ensuring the execution of local AFAP policy guidance/operating procedures. 5.5.1 AFAP Objectives. Contractor shall be responsible for the following objectives: 5.5.1.1 At least 120 calendar days before the date of the Focus Group meeting, Contractor shall be responsible for obtaining participants to attend the AFAP Focus Group by developing a marketing plan that publicizes the AFAP Focus Groups/Symposium (Two-Day Conference) to the Monterey m ilitary community, and obtaining quality of life issues/concerns from the members of the military community by attending community meetings/briefings, and providing issue forms for participants to fill out. 5.5.1.2 At least 50 calendar days from the date o f the Focus Group meeting, Contractor shall ensure that the appropriate proponent office(s) submits their recommendations for resolution of the issues raised during the Focus Group meeting. Contractor shall obtain resolution for past community issues that have been presented at the AFAP Steering Committee by coordinating/communicating with military agencies on their solution to the issues presented. 5.5.1.2.1 Contractor shall assist ACS in hosting facilitator/delegate training. 5.5.1.2.2 Contractor shall al so attend this training that is provide by ACS to become a facilitator, and become a facilitator for the AFAP Focus Groups/Symposium. 5.5.1.3 At least 30 calendar days before the Focus Group meeting, Contractor shall assist ACS in developing Delegate/Subje ct Matter Experts Packets that will be used for the participants at the Focus Group. AFAP Historical binders will show the required paperwork for delegate packets. 5.5.1.4 Within 30 calendar days of the Focus Group meeting, Contractor shall develop a foll ow-up list of the issues and resolution for the ACS Web Site from information that is obtained from the Focus Groups that the contractor attends, and submit it to the ACS Director. 5.5.1.5 Within 45 days of the conclusion of the Focus Group meeting, Contr actor shall provide an after-action report from the AFAP Focus Group/Symposium from the notes taken and the brief out provided by the AFAP participants, and submit to the ACS Director. 5.6 Installation AFAP Issue Update Book. 5.6.1 By the first working day of July, Contractor shall develop and implement an Installation AFAP Issue Update book that covers the top issues from the Focus Group/Symposium. 5.6.2 Contractor shall review documents and provide verification or feedback to the community on status of is sues and their progress. 5.6.3 Contractor shall prepare the AFAP Issue Update Book and ensure distribution to the community through ACS. 5.7 AFAP Resource Libra ry. Contractor shall maintain the AFAP resource library that includes as a minimum, the most recent HQDA AFAP Conference Brochure, AFAP Installation Handbook, and the HQDA Issue Update Book. 5.8 Records maintenance. 5.8.1 Contractor shall develop procedur es for the maintenance of all records. 5.8.2 These records/files will be subject to Government review at all times during the performance of the contract. 5.8.3 The files/records will become the property of the Government and shall be turned over to the AC S Director upon the completion or termination of the contract. 5.9 ACS Activities Celebrating Military Childrens Day, ACS Installation Volunteer Ceremony, Military Spouse Day, and AFTB Recognition Day. Contractor shall assist ACS in the execution of these community events to include recruiting volunteers, assisting with publicity, communicating/corresponding with units and outside organizations to achieve the objectives of the event. 5.10 Contractor shall comply with the Department of the Army Draft Accred itation Standards and any other resource files necessary to comply with this contract. 6.0 DELIVERABLES. See Paragraph 5.0, Technical Requirements. 7.0 SUPPORTING INFORMATION. 7.1 Qualifications of Contractor Personnel: Contractor shall possess as a minim um a high school degree; a BS/BA degree or 2-year degree is highly recommended. A copy of the diploma or degree achieved shall be submitted with a copy of the proposal. 7.2 Location of Work/Hours of Work: 7.2.1 Contractor shall primarily perform his/her services at the ACS Office, Building 4260, Ord Military Community. 7.2.2 Contractor may be required to perform his/her services approximately 10% of the time at an independent location. 7.2.3 Contractor shall perform his/her services during the normal hour s of 0800 to 1700 hours, with an hour for lunch, Monday through Friday. 7.2.4 Contractor shall be required to attend and participate in certain special programs/events which may be offered during after duty hours and located at different Government facilit ies. 7.2.5 Contractor shall not be required to provide services on designated Federal Holidays (Veterans Day, Thanksgiving, Christmas, New Years, Martin Luther King Day, Memorial Day, Presidents Day, Labor Day, Columbus Day, and Fourth of July). 7.3 Gover nment Furnished Property/Equipment: 7.3.1 The Government shall provide office space in the ACS facility, Building 4260, Ord Military Community Center, Seaside, California. 7.3.2 The Government shall also provide a desk, chair, computer, typewriter, use of copying machine, and all office supplies necessary to perform the required services and documents. 7.3.3 The Government shall furnish postal and installation distribution services for all official items. 7.4 Contractor-furnished Property/Equipment. The Gov ernment shall not be responsible in any way for damage to the Contractors supplies, materials, equipment, or the Contractors personal belongings due to fire, theft, accident, or other disaster. 7.5 Government-provided Training. ACS will provide four (4) hours of orientation on the focus of each of the above programs at no cost to the Contractor. Training will produce a working knowledge of the local AFAP process and the goals of the above listed programs. 7.6 Contractor Personnel: 7.6.1 Contractor is requ ired to be able to lift at least 25 pounds, catch, bend, stoop, reach, and have the ability to climb stairs. 7.6.2 Contractor is expected to work independently, without direct Government supervision. However, coordination with the ACS Director and the ACS personnel is considered essential to the successful completion of this contract. 7.6.3 Employee Identification: Contractor shall wear at all times a contractor-provided name badge/tag on his/her exterior clothing above the waist. The name badge/tag must co ntain the Contractors name and title. 7.7 Payment: 7.7.1 Contractor shall submit a copy of his/her monthly invoice to the Director of ACS or his/her representa tive for certification of hours worked. 7.7.2 For verification of services performed, Contractor shall be required to sign-in prior to starting work. The sign-in sheet will be located at the ACS Office. 7.7.3 If the Contractor cannot report to work due to illness or personal reasons, he/she shall call the ACS Director within one hour after the start of normal duty hours. 7.8 Quality Assurance: 7.8.1 Government personnel may conduct unannounced inspections for compliance. 7.8.2 Government personnel conducti ng inspections include, but are not limited to, the Contracting Officer and/or the Director of ACS or his/her representative. 7.8.3 Should the Contractor not concur with any inspection findings, he/she shall state, in writing, to the Contracting Officer, a ny area where he/she does not concur. Department of Labor Wage Determination No. 2005-2049 Rev 1 dated 9/19/06 applies to this acquisition.
