- Provide firm fixed rental pricing for equipment, supplies and services per patient per month for each of the VAMC(s) in VISN 20 by area of responsibility/jurisdiction for home oxygen supplies and services. Set-up is defined as delivery of prescribed Home Medical Equipment (HME), orientation of vendor's services, equipment assessment, patient education, basic home safety review and scheduling of ongoing deliveries and preventative maintenance inspection (PMI) service appointments. Contractor shall provide continuous oxygen services and supplies for those patients on travel throughout the United States including Alaska and Hawaii through local/and or national affiliates. Veterans are responsible for oxygen supplies and services while in transit via air. The contractor shall be responsible for providing services to the veterans once they reach their final destination. Some airlines will allow certain types of oxygen devices on their flights. It is the responsibility of the veteran to inquire about these devices and if available from the contractor, they can rent the devices at their own expense. The contractor's Respiratory Care Practitioner (RCP) must develop the Plan of Service, in accordance with the VA developed Plan of Care as well as coordinate with the appropriate clinical staff at each VA. No alteration to the prescription will be permitted without written prior approval of the Contracting Officers' Technical Representative (COTR) or the COTR's designated representative. Contractor shall replace all appropriate VAMC authorized (VA and contractor-owned) equipment presently located in homes of beneficiaries with his/her own equipment as soon as possible after award, but not to exceed 45 days. The change shall include delivery, setup instruction, and shall be accomplished with the least disruption in service to the VAMC beneficiary as possible. Cost will be prorated upon setup and discontinuance with equipment pickup no more than one billing cycle. All services will be performed in the veteran's place of residence or in a nursing home housing the veteran. The area of service shall be VISN 20 catchments' areas. The equipment needs are subject to change as determined by the prescribing physician. Please submit an offer for each geographic area you wish to be considered for award. Offerors shall have an established office physically located within one of the counties of the geographic area (s) they are submitting an offer on. The geographical boundaries described herein have been determined as the areas of responsibility/jurisdiction for each local VA Medical Center. Offeror will indicate specific area or areas of interest by submitting offer under the Schedule of Items as listed above. The Offeror under this solicitation cannot separate facilities incorporated under a health care system.
