- PLACE OF PERFORMANCE: Buffalo VA Medical Center, Buffalo NY 14215 United States AGENCY: Healthcare Network Upstate New York, Jonathan Aikin 2-90/NCA, 3495 Bailey Ave. Buffalo, NY 14215 POC: Contracting Officer - Jonathan Aikin, Ph :( 716) 862-7890, Fax :( 716) 862-8893 This is a combined synopsis/solicitation for commercial items prepared in accordance with the Federal Acquisition Regulation (FAR) sub part 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, proposals are being requested and THERE WILL BE NO WRITTEN REQUEST FOR QUOTATION ISSUED. The solicitation number for this Request for Quotation is VA528-07-RQ-0053. This solicitation is 100% set aside for Small Business. This solicitation is issued under NAICS Number 335911, size standard 500 employees. The Government intends to award a Firm fixed price contract under simplified acquisition procedures. Contractors' quotations are being requested and response to requirements listed above shall be submitted on contractor's letterhead or quotation form and must address all requirements listed and must contain the name, title and signature of person authorized to submit quotes on behalf of the contractor. DESCRIPTION: 98 EACH MODEL Alcad MC165P rated @ 161 Ah nominal Ni-Cad batteries; 1 EACH 130VDC Microprocessor Controlled Battery Charger (batteries are currently mounted to a two tier rack that measures 96" Length x 20" wide). 1 each spill containment system with flame-retardant neutralizing & absorbing pillow. BRAND NAME OR EQUAL MUST SUBMIT ALL MANUFACTURER'S SPECIFICATION. All quotations must include removal and recycling of all old Ni-Cad batteries, supervision, labor, installation and materials to install new Ni-Cad batteries, charger and spill containment systems to include all interconnecting hardware and cabling to mount on existing rack. Delivery, which shall be FOB Destination to VA Medical Center, Buffalo, NY shall occur within 60 days after award of the purchase order.  Offerors responding to this announcement shall provide all information contained in the Federal Acquisition Regulation (FAR) Clause 52.212-1, Instructions to Offeror-Commercial Items. As stated in this clause, the Government intends to evaluate offers and award a contract without discussions with offerors. Therefore, the offeror(s) initial offer should contain the offeror(s) best terms from a price and technical standpoint. However, the Government reserves the right to conduct discussion if later determined by the Contracting Officer to be necessary.  The provision at 52.212-2, Evaluation-Commercial Items, applies to this solicitation.  Paragraph (a) of FAR 52-212-2 is amended to read: "Lowest price technically acceptable". Offerors shall include a completed copy of provision 52.212-3, Offeror Representations and Certification--Commercial Items. An authorized representative of the offeror must sign these representations and certifications, and FAR 52.211-6, Brand Name or Equal. The Offeror shall also provide its DUNS number and Tax Identification number. The resulting firm fixed price award will incorporate the requirements of the clause at FAR 52.212-4, Contract Terms and Conditions--Commercial Items. Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercial Items, applies to this acquisition.  As of October 1, 2003, all Contractors, both large and small, are required to register in the Central Contractor Registration (CCR) database before an award.  Contractors are responsible for the accuracy of the information added to CCR.
