- Combined Synopsis/Solicitation HHM402-07-Q-0042 issued by the Virginia Contracting Activity (VaCA), on behalf of the Defense Intelligence Agency (DIA), to award a firm fixed price contract for a base period plus two option years, to acquire individual gym memberships in Arlington VA for a best estimated quantity of 26 employees located at 2451 Crystal Drive (Crystal Park 5) and 134 employees located at 3100 Clarendon Blvd. We are seeking facilities within 0.25 miles of each location, accessible by foot within a 10 minute walking radius, and that can support a military/government contract with flexible plans and rates. Specific Contract Requirements: (A) Period of Agreements. The period of the contract between DIA and the selected fitness facility(s) shall be a base period of eight months plus three option years. (B) Membership Privileges. DIA members who establish memberships with the selected facility(s) shall, as members, have at a minimum, full use of the facility(s) at the closest location during normal operating hours. (C) Eligibility. DIA will retain the right and responsibility to verify each individual's eligibility, will grant that individual the authority to use the contract, and will coordinate and control the memberships with the selected fitness facility(s) via the Contracting Officer (CO) and/or a designated representative of the CO, as necessary. During the initial 30 days of the contract, DIA will be responsible to provide the names of eligible members who shall have the opportunity to establish memberships with the selected facility(s). (E) Dues Payment and Subsidy. Monthly dues payments will be paid in agreement with terms agreed upon by the facility(s) and DIA contracting representative(s). All membership payments will be 100% subsidized by the Government. (F) Additional Memberships. The contract with the selected facility(s) shall allow new eligible personnel to begin memberships at any time, at the monthly rate established within the contract. (G) Temporary Dues. The contract with the selected facility(s) shall incorporate a method to allow employees identified by DIA as being in a temporary status to use the selected facilities at a reasonable per usage charge for the entire period of the military member's assignment in the area. (H) Transfers. DIA employees that transfer, due to official Government orders, as determined by DIA, during the period of their membership, shall have their membership cancelled, but the facility will retain the remaining balance for the full month of termination. Any employee that transfers temporarily for a period of less than 6 months shall have his/her membership put into an inactive status by the fitness facility upon official notification of said change of status. (I) Current Members. Eligible DIA members who already maintain current memberships at the selected facility(s) at the signing of this contract will be entitle to transfer their membership to this contract. (J) Guest Passes. The selected facility(s) shall make provisions for DIA to acquire 1-day guest passes for the purpose of oversight and observation of military member(s) participation. These passes will be distributed to military members through the Contracting Officer (CO) or a designated representative of the CO. (K) Usage Reports. The selected facility(s) shall provide DIA/HCH-3 with monthly usage reports to include frequency of use by individuals. This solicitation supports a requirement for commercial services prepared in accordance with Federal Acquisition Regulation (FAR) Subpart 12.6 and supplemented by additional information included herein. This announcement is the only request for Quotation (RFQ) to be issued; electronic quotations are required. Mandatory FAR clauses and provisions are: 52.212-1 Instruction to Offerors-Commercial Items (Sept 2006); 52.212-2 Evaluation-Commercial Items (Jan 1999) (a) Evaluation factors, in descending order of importance are (i) technical compliance with the requirements of this solicitation, (ii) past performance, and (iii) price. Technical and past performance, when combined, is 80%; 52.212-3 Offeror Representation and Certifications-Commercial Item (Nov 2006) Alternate I (Apr 2002); 52.212-4 Contract Terms and Conditions-Commercial Items (Sept 2005); 52.212-5 Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commercial Items (Nov 2006). Mandatory Defense Federal Acquisition Regulation Supplement (DFARS) clauses and provisions are: 252.212-7000 Offeror Representations and Certifications-Commercial Items (Jun 2005); 252.212-7001 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items (Nov 2006); and 252.209-7001 Disclosure of Ownership or Control by the Government of a Terrorist Country (Oct 2006). Responses to the RFQ are due by 2:30 P.M., Wednesday, 23 January 2007. Vendors may submit their quotes on electronic company letterhead stationery, and/or on Standard Form (SF) 1449. Responses must be submitted electronically (Internet) to Michael.VanDyke@dia.mil. Vendors must quote their prices according to the following Contract Line Item Numbers (CLINs). BASE PERIOD: 1 February 2007-30 September 2007. CLIN 0001 Membership-Clarendon comprised of sub-CLINs 0001AA Processing/Application Fee per individual (Not to Exceed 140 each); 0001AB Monthly Dues per individual/per month (Not to Exceed 1120 months). CLIN 0002 Membership-Crystal Park 5 comprised of sub-CLINs 0002AA Processing/Application Fee per individual (Not to Exceed 30 each); 0002AB Monthly Dues per individual/per month (Not to Exceed 240 months). OPTION PERIOD ONE: 1 October 2007-30 September 2008. CLIN 1001 Membership-Clarendon comprised of sub-CLINs 1001AA Processing/ Application Fee per individual (Not to Exceed 140 each); 1001AB Monthly Dues per individual/per month (Not to Exceed 1680 months). CLIN 1002 Membership-Crystal Park 5 comprised of sub-CLINs 1002AA Processing/ Application Fee per individual (Not to Exceed 30 each); 1002AB Monthly Dues per individual/per month (Not to Exceed 360 months). OPTION PERIOD TWO: 1 October 2008-30 September 2009. CLIN 2001 Membership-Clarendon comprised of sub-CLINs 2001AA Processing/ Application Fee per individual (Not to Exceed 140 each); 2001AB Monthly Dues per individual/per month (Not to Exceed 1680 months). CLIN 2002 Membership-Crystal Park 5 comprised of sub-CLINs 2002AA Processing/Application Fee per individual (Not to Exceed 30 each); 2002AB Monthly Dues per individual/per month (Not to Exceed 360 months). OPTION PERIOD THREE: 1 October 2009-30 September 2010. CLIN 3001 Membership-Clarendon comprised of sub-CLINs 2001AA Processing/ Application Fee per individual (Not to Exceed 140 each); 3001AB Monthly Dues per individual/per month (Not to Exceed 1680 months). CLIN 3002 Membership-Crystal Park 5 comprised of sub-CLINs 3002AA Processing/Application Fee per individual (Not to Exceed 30 each); 3002AB Monthly Dues per individual/per month (Not to Exceed 360 months). An award from the RFQ may be made to the vendor whose quotation offers the Government the Best Value, which is not necessarily the quotation with the lowest realistic cost or highest technical score. If the competitive range is considered essentially equal in terms of technical capability, the Government reserves the right to award to the offeror with the lowest price. The Government may award a contract based on quotes received, without discussions with offerors.
