Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The contractor shall provide all management, labor, material, equipment, certifications and supplies necessary to provide mobile dental services at the following location, Department of Ve terans Affairs  TOGAS, Augusta, Maine for the Maine Army National Guard in accorance with the Statement of Work (SOW). The mobile dental service must meet OSHA standard with the appropriate shielding and not just be portable dental equipment in the back of a box van. Date of Service is 3-4 March 2007 for 150 soliders. Statement of Work (SOW) is as follows:1. BACKGROUND: Oral health is vital for force health protection and failure to achieve a standard of level of oral health will preclude a soldier from deploying. Dental Readiness is a command responsibility. DoD established a uniform dental classification in HI Policy 02 -11. DoD policy requires dental screening examinations to be accomplished and documented on 90% of service members. Members of the Maine Army National Guard shall receive an annual dental screening examination to determine their dental classification. The results of that screening examination shall be entered into the DENCLASS database no later than 30 days from the date of examination. The DENCLASS database updates the Army Medical Protection System, MEDPROS. 2. OBJECTIVES AND SCOPE: The objective of this State of Work is to conduct and complete the requirement of Dental Readiness on each Soldier in the Maine Army National Guard utilizing a mobile dental facility. Those requirements include: " Clinical evaluation by a licensed dentist of the oral cavity using a visual, clinical exam supported by bitewings taken by dental technicians and the panoramic x-ray taken by dental technicians if clinically indicated in the dental record. " Each exam will be entered into the DENCLASS reporting system and annotated on the SF 603 (Health Record  Dental) and the SF 5570 (Health Questionnaire for Dental Treatment) of the Soldiers dental records. " Personnel performing the dental screening examination have a moral and ethical obligation to inform the Soldier if they observe or are apprised of any sign or symptom for which the Soldier should obtain further evaluation or care. " A dental screening examination identified deficiencies but does NOT include the dental treatment required to restore those deficiencies. 3. DEFINITIONS: None 4. GOVERNMENT-FURNISHED PROPERTY AND INFORMATION: TOGUS VA will provide a workspace for the Dental team to perform the Dental Readiness requirements to include parking area for the Mobile Dental unit. 5. CONTRACTOR FURNISHED ITEM: Contractors will provide all items and licensed personnel needed to perform the Dental Readiness requirements, any employee benefits, Vacation days, Paid Holidays and Sick Leave plan. 6. SPECIFIC TASKS: Perform 150 required dental examinations to include bitewings and panoramic x-ray if clinically indicated in support of the annual physical exams scheduled for 3 & 4 March 2007. Each exam will be entered into the DENCLASS reporting system and annotated on the SF 603 (Health Record  Dental) and the SF 5570 (Health Questionnaire for Dental Treatment) of the Soldiers dental records. Setup of contractors dental equipment is to be completed by 2 Mar 2007 at 1700. Setup of contractors dental equipment is the sole responsibility of the contractor. Breakdown of the contractors dental equipment is the sole responsibility of the contractor. As required provide timely feedback and work progress/ status to number of Dental examinations completed to the leader of the Medical Team and the leader of the Mobilizati on team. Electronic entry of data into the DENCLASS database is required within 5 days of completion of the Dental Screening mission. Contractor shall be licensed on any services rendered and equipment operated during mission. 7. APPLICABLE DOCUMENTS: None 8. PLACE OF PERFORMANCE: TOGUS VA Hospital, TOGUS, maine 9. PERIOD OF PERFORMANCE: 3-4 March 2007 10. OTHER CONDITIONS/ SECURITY: a. Safety: The contractor has established a safety and health program in accordance with the Federal Occupational Safety and Health Standard. The contractors safety program includes equipment training, occupational safety, health, and fire protection gu idance to ensure the protection and health of all work personnel. Government property will also be protected during the period of time the property is under the control or possession of the contractor. b. Performance work statement is UNCLASSIFIED and under HIPAA regulations. Contractor shall notify the contracting officer if requirement exists for secret classification and circumstances requiring secret classification for Readiness impact reporting. 11. ACCEPTANCE PLAN: All work performed, all materials and tasks data to be submitted under this task will be made available to be inspected for compliance with the SOW and accepted by the COR or authorized representative as specified herein.The Government will award a contrac t resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: The best value proposal will be determined by evaluating the contractors offer based on Price and Past Performance and the following technical factors in descending order of importance: 1) Contractors ability to meet the requirements of Section 1 of the Statement of Wor k. 2) Contractors experience with and knowledge of Dental operations/regulations of the Army National Guard. 3) Contractors ability to meet 3 day response time. The technical evaluation factors and past performance, when combined, are approximately equal i n value to cost or price. Offerors shall ensure that the above factors are listed in their proposals. The provision at 52.212-4, Contract Terms and Conditions Commerical items, applies to this acquisition. Offerors shall include a complete copy of the prov isions at FAR 52.212-3, Offeror Representations and Certifications Commerical items with its proposal. The provisions at FAR 52.212-5, Contract Terms and Conditions required to implement statutes or Executive Orders commercial items, are included and incor porated by reference.
