Description
SCHEDULE Contractor shall furnish all labor, tools, and supplies necessary to perform timely and effective preventive maintenance service and unscheduled repairs and service calls on Material Handling Equipment located at the Defense Reutilization and Marketing Office, RIPL Blue Grass, Blue Grass Army Depot, Building 266, 1091 Kingston HWY, Richmond, KY. Period of performance shall be from date of award through September 30, 2007 with 4 one-year options. NOTE: The 4 EA quantity for is for the four preventative maintenances that are required for each piece of equipment per year. Parts used for these repairs shall be billed at contractor?s cost. The following clauses also apply: 1. Insurance - Work on a Government Installation (SEP 1989) - FAR 52.228-5 2. Insurance (SEP 1989) DSCC 52.228-9C02 The kinds and minimum amounts of insurance required by FAR 52.228 - 5 are as follows: TYPE MINIMUM AMOUNT a. General Liability Insurance $500,000 per occurrence Bodily Injury b. Automobile Liability Insurance $20,000 per occurrence Property Damage $200,000 per occurrence Bodily Injury $500,000 per occurrence 4. This contract is subject to the Service Contract Act of 1965, As Amended (May 1969). Department of Labor Wage Determination Number 2005-2221, Revision 2, applies to this procurement. This may be viewed at www.wdol.gov. 5. Option to Extend Services (AUG 1989) - FAR 52.217-8 The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing Labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the contractor within the period specified in the schedule. 6. Option to Extend the Term of the Contract (MAR 1989) - FAR 52.217-9 (a) The Government may extend the term of this contract by written notice to the Contractor within fourteen days before the contract expires; provided, that the Government shall give the Contractor a preliminary written notice of its intent to extend at least 14 days before the contract expires. The preliminary notice does not commit the Government to an extension. (b) If the Government exercises this option, the extended contract shall be considered to include this option provision. (c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 5 years. STATEMENT OF WORK Description/Specifications/Work Statement Scope: The work shall consist of furnishing all labor, tools, supplies, oil, grease, filters, etc., required to perform timely and effective preventive maintenance (PM), and repair service on an as needed basis for material handling equipment located at the RIPL Blue Grass, Blue Grass Army Depot, Building 266, 1091 Kingston HWY, Richmond, KY. Quarterly Maintenance Inspections Furnish all material, labor and equipment to perform preventive maintenance on a quarterly basis for construction and material handling equipment located at RIPL Blue Grass, Blue Grass Army Depot, Building 266, 1091 Kingston HWY, Richmond, KY. As part of the inspection, the contractor shall perform the following: 1. Contractor shall perform visual inspection of engine assemblies for leaks, wear and damage, belts, chains, tire wear and pressure, loose of unsecured components, lift chains, exhaust systems, brakes, engine oil level, hydraulic lines, fittings and fluid levels. 2. Contractor shall check radiator, radiator cap, radiator core, gaskets and hoses for leakage; correct mixture to prevent freezing during extreme cold weather; and change anti-freeze annually during the month of September. 3. Contractor shall change oil and filters and lubricate in accordance with manufacturer?s maintenance requirements. 4. Contractor shall inspect lifting devices at periodic intervals to ensure compliance with the Occupational Safety and Health Act (OSHA). Contractor shall notify the contracting officer when replacement is required and shall obtain written approval prior to proceeding with the replacement. Contractor shall ensure replacement. MAINTENANCE REPAIR SERVICE Maintenance and repair of construction and material handling equipment on an as needed basis. Contractor shall respond within 12 hours after notification of an equipment malfunction/breakdown. Labor shall be at hourly rate specified in the contract. Parts and materials to be invoiced at cost. Invoices The contractor shall be required to submit invoices immediately following services performed. Invoices must include the contract number and the equipment serviced identified by DLA number. All original invoices should be sent to DRMO Wright Patterson for certification at the address listed below: DRMO Wright Patterson Attn: Lisa Vest P.O.
