Description

The VA Healthcare Network Upstate New York is seeking to contract with a qualified contractor to provide Vascular Interventional Radiology Services to beneficiaries of the VA Healthcare Network Upstate New York (VISN 2), 113 Holland Avenue, Albany, NY 12208. The Contractor shall provide a Board Certified and CAQ Sub-Specialty Certified Interventional Radiologist to provide Pre-Authorized Vascular Interventional Radiology Services at the contractor?s site utilizing contractor owned and maintained equipment and the contractor will also provide On-Call Services in accordance with the terms and conditions of the contract. The proposed contract period will be for the period April 1, 2007 (or date of contract award) through March 31, 2008 with two (2) option year renewals. The proposed contract will be a firm-fixed price contract.
