Description
1. Scope: a. Background and purpose: Current health promotion efforts of Shiprock Service Unit and local tribal programs are often limited to the immediate Shiprock Area. There are very few health promotion or wellness activities available to residents of the 24 outlying chapters within the Shiprock Service Unit. Efforts of the Student Leadership have been limited to the immediate Shiprock area because of funding constraints. The lack of opportunities for exercise and wellness activities is a major factor in the development of increasing rates of chronic illness among Navajo people. Increasing rates of chronic diseases among Navajo people in Shiprock Service Unit are often related to changes in exercise and nutrition habits and patterns over the last 50 - 100 years. The lack of availability of wellness and exercise facilities is a significant factor in the current epidemic of obesity faced by Navajo people. In addition, current high death rates of motor vehicle crashes and intentional injuries related to high rates of alcohol and substance abuse among local young people have been related to lack of positive available opportunities. Contractor will also provide a series of educational activities for students during the summer months and fall period. 2. Requirements a. Tasks As requested by SRSU Health Promotion, under the terms of this BPA, 1. Program development and implementation of Shiprock Community Wellness Center program in Shiprock Service Unit. Wellness Center will provide fitness opportunities on a regular basis to all interested Shiprock SU patients ages 16 and up, 2. Assist with all aspects of wellness canter program including public relations, chapter coordination, advertising, incentives, rules, safety, liability, and security issues. 3. Program will work with local community and schools to identify resources and implement community fitness promotions including leagues, after-school recreations, sport camps, life skills enrichment, and prevention interventions. 4. Program shall provide support for the development of HPDP promotional events relating to injury prevention, violence prevention and other HPDP related projects. 5. Contractor shall provide a series of workshops and youth activity days during the year of 2006 including a nationally sponsored Alcohol/Substance Abuse prevention program, educational field trips, wellness conference and sports training, alcohol and drug free events. 6. Contractor shall provide access to weight room and fitness facilities to all current Shiprock Hospital employees and their immediate families, spouse and their children. Access will be by employees showing membership card. 7. Contractor shall provide direct consultation to PHS employees in development of their own fitness programs including design of circuit training. 8. Contractor will set up and maintain hike and bike program for up to fifty (50) children. 9. Contractor will have meeting facility available for nutrition education classes. b. Reporting Requirements Contractor shall provide monthly activity summaries to Director of Community Health, Shiprock Service Unit. c. Deliverables None d. Schedule Upon award of contract vendor will initiate project and carry out over seven (7) months after the date of contract award. e. Period of performance Project is estimated to take seven (7) months. f. Government property No government property to be used. 3. Quality Assurance Provisions JASL will meet during the project period with Director of Community Health Services to review project, progress, problems, and adequacy of services provided. Interim reports of services provided will accompany monthly invoices. 4. References Development of this project has previously been approved and supported by Shiprock Service Unit Health Promotion Program. Summary Contractor shall provide consultation services in the development of nutrition/fitness programs for use in community, family and youth health education and health promotion efforts of Shiprock Service Unit Navajo Area Indian Health Service.
