- This is a combined synopsis/solicitation for services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. Request solicitations from cindy.l.walton@us.army.mil. STATEMENT OF WORK MOBILE OCCUPATIONAL MEDICAL SURVEILLANCE EXAMINATIONS 1. PURPOSE 1-1. The purpose of the Kentucky Army National Guard (KYARNG) Mobile Occupational Medical Surveillance Examination Program is to provide job-related medical surveillance examinations and occupational health services to approximately 275 KYARNG fu ll-time support (FTS) or temporary personnel who are exposed or potentially exposed to health hazards in the work environment, or who are assigned to positions requiring specific standards of physical fitness. This Statement of Work does not cover the Sta te of Kentucky employees, civilian contractors, or the KYARNG part time soldiers (known as M-Day soldiers). The Government may evaluate the quality of professional and administrative services provided, but retain no control over the medical professional a spects of services rendered (e.g. testing procedures, etc). The contractor will provide all labor, tools, supervision and supplies to conduct mobile medical surveillance/ occupational health services for the KYARNG. 1-2. To conduct medical surveillance examination/occupational health services at (3) KYARNG work-sites/locations listed below; proposed start date: NLT 1 May 2007. SITE/LOCATION NO. To establish, maintain, and protect occupational health records against unauthorized access pursuant to the Privacy Act of 1974 and Department of the Army Policy (5 USC 552a). 1. PERFORMANCE PERIOD 2-1. Period of contact shall be from the time of the contract award through 1 September 2007. 3. KENTUCKY ARMY NATIONAL GUARD PROJECT OFFICER 3-1. Contractor must ensure all procedures are conducted in accordance with (IAW) applicable American Board of Occupational and Environmental Medicine (ABOEM), American Association of Occupational Health Nurses (AAOHN), U.S. Department of Labor, Occupational Safety and Health Administration (OSHA), and National Institute for Occupational Safety and Health (NIOSH) Standards. 5-2. Contractor must ensure all clinical laboratory services are performed by a laboratory licensed through the U.S. Department of Health and Human Services (DHHS) Health Care Financing Administration pursuant to the terms of the Clinical Laborato ries Improvement Act of 1967 (42 U.S.C. 263a) and the College of American Pathologists. 5-3. Contractor must provide a Board Certified Occupational Medicine Physician(s) and Audiologist to conduct job-related medical surveillance physical examinations and occupational health consultation services for approximately 275 KYARNG employees at three (3) KYARNG work-sites. L icense must be available. 5-4. Contractor will arrange for initial coordination of services with the Project Officer within fifteen (15) working days after award of the contract. 5-5. Contractor will arrange for the opportunity to consult with a representative of the company by telephone to coordinate or resolve issues that might arise before, during, and after the testing period. 5-6. A comprehensive review and evaluation will be performed within thirty (30) days following the conclusion of testing. The KYARNG will contact and schedule a follow-up review and evaluation meeting to discuss positive and negative aspects of t esting. 5-7. Only licensed medical technicians will perform medical surveillance screening/testing elements. Personnel performing audiograms will be certified by the Council for Accreditation in Occupational Hearing Conservation (CAOHC). Personnel cond ucting spirometry will be certified through a program approved by the National Institute for Occupational Safety and Health (NIOSH). License and certifications must be available. 5-8. Instrumentation must meet or exceed appropriate performance standards, and accuracy must be verified IAW current Occupational Medicine Certification standards. 6. MEDICAL SURVEILLANCE 6-1. 6-2. Occupational medical surveillance will be conducted in two (2) phases. Phase one will consist of testing only. And phase two will consist of examinations only. 6-2. Occupational medical surveillance will consist of a physical examination to include: complete occupational health history, head-to-toe physical examination, dermatological exam, anthropometric measurements, vital signs, vision screen, complete blood count (CBC) with differential, blood chemistries (22 item panel that includes HDLs and LDLs), on board urinalysis, audiometry, and respirator clearance. 6-3. Based upon age and occupational exposures, additional testing may include: a prostate specific antigen (PSA) test, blood lead with zinc protoporphyrin, and 1-view (PA) chest x-ray with B-reader, C-Reactive protein analysis. 7. PERSONAL PROTECTIVE EQUIPMENT (PPE) 7-1. Contractor shall furnish and assure health workers are provided with appropriate PPE such as, but not limited to, gloves, protective eyewear etc., where there is a potential for exposure to blood or other potentially infectious materials. 7-2. Contractor will ensure each health care worker complies with the requirements of 29 CFR 1910.1030 Bloodborne Pathogens Standard. 8. INFECTIOUS WASTE DISPOSAL 8-1. Disposal of all infectious waste must be in accordance with local, state, and federal regulation and disposed of by the contractor. 8-2. Puncture resistant sharps containers must be readily accessible to health care workers and located in all treatment areas where sharps could potentially be used. 9. MOBILE UNIT CONFIGURATION 9-1. Medical surveillance testing, (9.0 days) is to be accomplished via mobile unit designed to insure complete individual privacy. Features of the unit should include, but are not limited to, a climate-controlled atmosphere, patient waiting/hold ing area for registration, changing room, on board restroom for urinalysis, testing/phlebotomy station, individual audiometric testing booth, 9-2. Mobile units must be equipped with a self-contained power source. Shore power will not be provided. 10. PHYSICIAN SERVICES 10-1 Physician(s) will provide an on site examination of the (FTS) employees as they complete mobile testing. 11. MEDICAL REPORTS AND RECORDKEEPING 11-1. Th e contractor will ensure two (2) copies of the employee examination findings are furnished. One copy is to be sent to the KYARNG Project Officer, and one copy is to be mailed directly to the individual employees home of record. Medical examination resul ts shall include all diagnostic tests, graphs, charts, reports, questionnaires, waivers, employee consent forms, physical findings, and examination results. The contractor will provide the KYARNG Project Officer with a hardcopy of these results within thi rty (30) working days of completion of testing. Results of medical surveillance testing which are questionable and/or of critical significance will be communicated directly, and immediately, to the KYARNG Project Officer by telephone, facsimile, and/or ex press mail. 11-2. At a minimum, all medical records are to be identified by the contractor with the employees name, social security number gender, job code, and the name of the employees workplace or facility. The contractor must ensure work/health history forms and medical reports are complete and legible (typed/printed). 11-3. Supplemental Forms. Employees will be instructed to complete a release form if required by the contractor. The Project Officer will provide the contractor with a Privacy Act Statement (DD Form 2005) for each employee to sign during the regi stration process. 12. AUDIOMETRIC TESTING 12-1. Meet all applicable Hearing Conservation Program requirements in accordance with Department of the Army Pamphlet (DA PAM) 40-501, Hearing Conservation, 10 December 1998, and the U.S. Army Center for Health Promotion and Preventive Medicine ( USACHPPM) Technical Guide (TG) No. 167A. 12-2. Test Parameters: The contractor will perform an air conduction audiogram a 500, 1000, 2000, 3000, 4000, and 6000 hertz (Hz) in both ears. Audiometric reports must contain: (a) the examiners name and CAHOC certification number; (b) the revie wers name (audiologist or physician), state license or certification number, or the Governments equivalent civilian job code (e.g. 0665-audiologist); and (c) the make, model, serial number and the most recent electroacoustic calibration date of the audio meter used. 12-3. Bioacoustic Calibration. Prior to the contract test date, the Contractor must provide documented proof of each audiometers last calibration date in accordance with the ANSI S3.6-1996 standard. The calibration date must be within one year of contract test dates. 12-4. During testing, hardcopies of the Audiometric data will be provided to the project officer twice daily. 13. EXAMINATION SCHEDULING 13-1. Coordination of scheduling will be handled in accordance with paragraph 5-4. All workdays include a one-hour lunch break. No testing will be performed between 1130-1230 hours. 13-2. In the event of a Contractor equipment failure, specific testing and/or parts of examinations that are not completed during the testing period will be completed at a scheduled time mutually convenient to the Contractor and the KYARNG. Unfin ished testing/examinations will be completed at no additional cost to the KYARNG. The option to schedule physicals not completed due to equipment failure, through a local occupational medical facility will be done at no cost to the KYARNG. The Contractor will be directly responsible to cover costs for completing these make-up exams. 14. INVOICING AND PAYMENT 14-1. Upon satisfactory completion of all services as outlined in this Statement of Work, the contractor may invoice for the services rendered. The invoice will show the actual quantity and type of service provided per site and per employee.
