Description: The VA Healthcare Network Upstate New York is seeking to contract with a qualified contractor to provide Nephrology Services for VA Healthcare Network Upstate New York (VISN 2), 113 Holland Avenue, Albany, NY 12208 by Board Certified/Board Nephrologist(s). The Contractor shall provide professional Nephrology Services in accordance with the terms and conditions of the contract. The proposed contract period will be for the period May 1, 2007 (or date of contract award) through April 30, 2008 with two (2) one-year option renewals.
