- (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation number VA-241-07-RQ-0089 is issued as a request for quotation (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-16. This solicitation incorporates one or more solicitation provisions by reference, with the same force and effect as if they were given in full text. The applicable North American Industry Classification System (NAICS) code for this acquisition is 561720. The small business size standard is $15.0 Million. (v) The contract will be a firm fixed price and include a base year and four one-year options. Contract line items are as follows: 0001. Base year - 6/1/2007 - 9/30/2007 (4) months. 0002. Option Year - One 10/1/2007 - 9/30/2008 (12) Months. 0003. Option Year Two - 10/1/2008 - 9/30/2009 (12) Months. 0004. Option Year Three - 10/1/2009 - 9/30/2010 (12) Months. 0005. Option Year Four - 10/1/2010 - 9/30/2011 (12) Months. (vi) The description of requirements for the items is as follows: The Contractor shall provide all labor, materials, equipment, transportation, supervision and disposal necessary to conduct cleaning of the Veterans Affairs Community Based Outpatient Clinic (CBOC) located at One Corporate Place, Middletown, RI (Newport County). The work includes: Daily (1) Empty trash and remove from office. (2) Empty medical trash and put in special bin in office. (3) Sweep and wash all examining room floors with germicidal. (4) Clean all sinks in examining rooms. (5) Clean kitchen area. (6) Replace paper and soap products as needed with VA furnished supplies. (7) Clean bathrooms thoroughly. Biweekly (1) Buff floors. Quarterly (1) Rug cleaning services to include continuous spot free service. As needed (1) Stripping and waxing of floors. (vii) Deliverables and acceptance of deliverables will be FOB destination at VA CBOC Middletown, RI. (viii) The provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. (ix) The provision at 52.212-2, Evaluation-Commercial Items, applies to this acquisition. The following text is added to Paragraph (a) of the clause: (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (1) Past Performance - The offeror shall furnish the names of three references, indicating contracts of similar magnitude and nature. References shall include a point of contact name and telephone number. The Government will evaluate the offeror's past experience for quality, response and service as reported by references furnished by the offeror. (2) Price - An offeror's price will be evaluated for price reasonableness and realism; to determine if an offeror's proposed price is realistic for the work to be performed and if it reflects a clear understanding by the offeror of the RFQ requirements. All evaluation factors other than cost or price, when combined, are approximately equal to cost or price. (x) The provision at 52.212-3 Alt. I, Offeror Representations and Certifications-Commercial Items applies to this solicitation. The contractor shall return a completed copy of these provisions with its offer. (xii) The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition. (b) The Contractor shall comply with the FAR clauses in this paragraph (b) that the Contracting Officer has indicated as being incorporated in this contract by reference to implement provisions of law or Executive orders applicable to acquisitions of commercial items: 52.219-6, Notice of Total Small Business Set-Aside; 52.222-3, Convict Labor; 52.222-21, Prohibition of Segregated Facilities; 52.222-26, Equal Opportunity; 52.222-36, Affirmative Action for Workers with Disabilities; Alternate II of 52.225-3; 52.225-13, Restrictions on Certain Foreign Purchases; 52.232-34, Payment by Electronic Funds Transfer--Other than Central Contractor Registration; (c) The Contractor shall comply with the FAR clauses in this paragraph (c), applicable to commercial services, that the Contracting Officer has indicated as being incorporated in this contract by reference to implement provisions of law or Executive orders applicable to acquisitions of commercial items: 52.222-41, Service Contract Act of 1965, as Amended; 52.222-42, Statement of Equivalent Rates for Federal Hires. (xiii) Service Wage Determination Number RI 05-2467 REV (02) is applicable to this acquisition. (xiv) The Defense Priorities and Allocations System (DPAS) and assigned rating are not applicable. (xv) There are no applicable Numbered Notes. (xvi) Quotations are due May 4, 2007 at 3:00 PM EST, mailed to the VA Medical Center (90C), Attention: Daniel Walker, 830 Chalkstone Ave, Providence, RI 02908. Faxed or emailed quotes are acceptable and encouraged.
