Description
DESCRIPTION/SPECIFICATIONS/WORK STATEMENT GEC PHYSICIAN SERVICES Provides assistance to management in strategic and programmatic direction relative to the provision of geriatric medical and health services for the Department of Veterans Affairs VA Medical Center, Manchester, NH 03104 for the period _____________ through ____________. The related services performed shall be as follows but not limited to: Long Term Care: 1. Directs and coordinates care on Long-Term care Units ensuring that the medical care provided is appropriate and adequate. 2. Assesses referrals for admission and determines clinical appropriateness for the level of services provided. 3. Assures that GEC medical staff functions are managed consistent with applicable standards and regulations and JCAHO accreditation requirements. Oversees the compliance of DNR orders with policy. Reviews and signs 30 day drug reviews. Responsible to review and/or write and sign discharge orders and discharge summaries. Documentation Oversees practice of PA-C. Co-signs H&Ps. Assesses all patients for change in condition. Initiates transfer to acute level of care with MD to MD communication, as necessary Assesses each resident monthly for first 90 days of admission then quarterly. Completes annual reviews. Renews orders: code status q 30 days, reatraints q 30 days, medication, other. Lab order entry. Attends Interdisciplinary Team Meeting for care planning twice weekly (three hours approximate for each session). Written hand-off communication to Urgent Care each Friday. 4. Collaborates with the GEC LSLM to ensure adequacy and appropriateness of GEC Service line's services. 5. Member of GEC Management Board and a regular (monthly) meeting for GEC Clinicians. Attends Executive Committee of the Medical Staff and is a member of the Local Pharmacy and Therapeutics committee. 6. Collaborates with the geriatric pharmacist to ensure best practices in the use of pharmacotherapeutics. 7. Responsible for onsite availability _40___ hours per week and on call availability for telephone consultation week days. Home Based primary Care: . Directs and coordinates care in HBPC ensuring that the medical care provided is appropriate and adequate. Attends weekly Interdisciplinary Team Meetings and oversees Care Planning, providing clinical input for all patient treatment plans at 30 and 60 day intervals and quarterly thereafter. Is readily available to team members for collaboration when medical or other problems arise. Provides medical administrative support such as signing forms requiring physician signature for the HBPC patient.
