Description

CHRONIC BRAIN INJURY REHABILITATION STATEMENT OF WORK Contractor shall provide residential treatment and rehabilitation services in a licensed community based environment designed exclusively for adults with brain injuries. The contractor shall provide housing, meals, 24 hour 7 day per week support or supervision along with the following therapies: recreation, social, corrective, cognitive and personal adjustment counseling. The contractor shall provide a CBIS(Certified Brain Injury Specialist) person. The contractor shall assist in transportation on medical outpatient visits to the VAMC Manchester, NH and shall be responsible for all associated transportation costs.. the VAMC Manchester shall be responsible for all medical and pharmaceutical services and costs. Contractor shall provide services structured for independent living in a highly supervised setting. Contractor shall provide all licenses related to this service. During the course of this contract it is possible that other stations may use this contract. The station will be responsible for payment and placement but will be added on when the supply to this office a obligated 2237 form
