- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this Request for Quotation is VA-69D-07-RQ-0387. This solicitation document incorporates provisions and clauses in effect through FAC 05-18. This solicitation is Unrestricted. The associated NAICS Number is 339113. The FSC Code is 66. VA Great Lakes Acquisition Center has a requirement for the following item(s): Quantity of (1) neo2000 Bluetooth Gamma Detection System, Code NEOBT $___________; Quantity of (1) Accessory Case, Part Number NPA71 $___________; Quantity of (2) AC Power Cord, Part Number NPA75 $____________; Quantity of (2) 14mm External Collimator, Part Number NPE14 $____________; Quantity of (1) Neoprobe Bluetooth Serial Port Adapter Kit, Part Number NPBAK, $__________; Neoprobe Bluetooth 14 mm probe Angled, Part Number NPB14 A, $____________; Price to include Operator's Manual Quantity of (1) and One Year Extended Warranty for neo2000 and Probe. The Manufacturer of this equipment is Ethicon Endo-Surgery, Inc.. Brand Name or Equal. Delivery, which shall occur within 45 calendar days of contract award, shall be FOB DESTINATION to WM S. Middleton VAMC, 2500 Overlook Terrace, Madison, WI 53705. The Contractor is responsible for all freight charges. The provision at 52.212-1, Instruction to Offeror--Commercial Items, applies to this solicitation. The provision at 52.212-3, Offeror Representations and Certification--Commercial Items, applies to this solicitation. The contractor shall return a completed copy of this provision with its quotation. A copy of the provision may be attained from http://www.arnet.gov/far/current/html/52_212_213.html or completed at https://orca.bpn.gov/. The resulting firm fixed price award will incorporate the requirements of the clause at FAR 52.212-4, Contract Terms and Conditions--Commercial Items. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercial Items, applies to this acquisition. The Offeror shall also provide its Commercial and Government Entity code, DUNS number, and Tax Identification number. As of October 1, 2003, all Contractors, both large and small, are required to register in the Central Contractor Registration (CCR) database before an award. Contractors are responsible for the accuracy of information added to CCR. To register, go to http://www.ccr.gov/. The Federal Contractor Program requires that any contractor receiving a contract from the Federal Government in the amount of $25,000 or more, or any subcontractor receiving a contract in the amount of $25,000 or more from such a covered contractor must file a VETS-100 Report on an annual basis.
