Description

1. SCOPE The Department of Veterans Affairs, Vocational Rehabilitation and Employment Division, Houston Texas Regional Office, is soliciting offers for an indefinite delivery - indefinite quantity (IDIQ) contract to provide Independent Living Assessment Services for its eligible veterans. The services are currently being performed; however, the performance period will expire on or about September 30, 2007. The Department of Veterans Affairs is currently in the process of establishing a regional contract as part of its National Acquisition Strategy (NAS) ; however award of this contract is not expected for up to six months after the expiration of the contract under which services are currently being provided. Therefore, the VR&E Division must award a contract to provide the services until the NAS for this region is awarded. Because of this the initial period of performance will be 6 months, however, the Government may extend the term of the contract for up to an additional 6 months. 2. SERVICES The services to be provided include: 1) Perform a complete and accurate assessment of the veterans interior and exterior environment to determine possible hindrances to successful independent living activities which thoroughly; 2) Assess the degree to which the veteran's present disabilities provide barriers to successful daily living; 3) Provide a comprehensive report detailing the findings of the assessment, including recommendations to resolve the difficulties or obstacles observed. 3. GEOGRAPHIC COVERAGE AREA The Contractor will be required to provide services for all of the following geographic areas: Houston, Texas Area Counties: Austin, Brazoria, Chambers, Colorado, Fort Bend, Galveston, Grimes, Harris, Houston, Jackson, Lavaca, Liberty, Madison, Matagorda, Montgomery, San Jacinto, Trinity, Walker, Waller, Washington, Wharton San Antonio, Texas Area Counties: Atascosa, Bandera, Bexar, Comal, De Witt, Edwards, Gillespie, Gonzales, Guadalupe, Karnes, Kendall, Kerr, Kinney, McMullen, Medina, Real, Uvalde, Val Verde, Wilson Corpus Christi, Texas Area Counties: Aransas, Bee, Calhoun, Goliad, Jim Wells, Kleberg, Live Oak, Nueces, Refugio, San Patricio, Victoria Rio Grande, Texas Area Counties: Brooks, Cameron, Dimmit, Duval, Frio, Hidalgo, Jim Hogg, Kenedy, La Salle, Maverick, Starr, Webb, Willacy, Zapata, Zavala Northwest Texas Area Counties: Brewster, Crockett, Kimble, Menard, McCulloch, Pecos, Schleicher, Sutton, Terrell Austin, Texas Area Counties: Blanco, Caldwell, Hays, Mason Beaumont, Texas Area Counties: Angelina, Hardin, Jasper, Jefferson, Nacogdoches, Newton, Orange, San Augustine, Shelby, Tyler 4. PERIOD OF PERFORMANCE It is anticipated that any resultant contract will be awarded for a 6-month base period, and may be extended for an additional 6 months. 5. ESTIMATED QUANTITIES The actual number of cases that the contractor will be assigned or managed will vary. However, based on previous services provided, the estimated number of cases 250. 6. SET-ASIDE This solicitation is being set-aside for Service-Disabled Veteran Owned Small Business concerns only. The applicable size standard for determining small business is $6,500,000 average gross receipts over a three (3) year period.
