- This requirement is within the Military Treatment Facility (MTF), Naval Hospital (NH), Pensacola, Florida, for Mobile Lithotripsy Services. The MTF desires an agency that will provide a lithotripsy mobile unit (equipment) and a qualified operator (Radiologic Technologist) one day per month. The Radiologic Technologist will work with a Government Urologist who will provide technical direction. The Contractor shall provide the Lithotripsy Mobile System and personnel to perform lithotripsy procedures one (1) weekday per month (e.g., Monday, Tuesday, Wednesday, Thursday, or Friday). Services will not be required on Federal holidays or on the day of observance. The Government will coordinate a delivery schedule with the contractor one month in advance for each 1-month period. The MTF anticipates approximately six to eight procedures per month. The Government will schedule appointments for lithotripsy service and confirm patient appointments not less than 48 hours prior to performance of the procedure. Upon each scheduled visit, the Contractor shall deliver, set-up, shut-down, and remove the mobile lithotripsy unit and shall complete the manufacturer?s routine quality control procedures for the mobile lithotripsy unit. The Government will schedule a Biomed inspection to be performed prior to every scheduled visit. The Government will perform all pre- and post-procedure duties, including escorting the patient to and from the MTF?s waiting area and procedural location (e.g., MTF?s main operating room or ambulatory procedure unit). The Contractor shall provide all support personnel who meet the qualifications as specified in the Scope of Work. The Contractor personnel shall operate the lithotripsy unit and related equipment independently and under the technical direction of the Government Urologist. The Contractor personnel shall recognize the need for and institute emergency measures should a patient require resuscitation procedures. Contractor employees shall demonstrate sufficient initiative, interpersonal relationship skills and social sensitivity such that they can relate constructively to a variety of patients and staff from diverse backgrounds. The following additional provisions and clauses apply: 52.212-2, Evaluation-Commercial Item, the Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the government, price and Past Performance considered. Technically acceptable will be evaluated based on the offeror?s demonstrated ability to perform the required services based on Past Performance. Offerors shall submit descriptive literature of the lithotripsy services they provide. Past Performance will be considered as the most important factor. Price will be evaluated to determine its fairness, completeness, and reasonableness as it relates to the services offered. 52.212-3, Offeror Representations and Certifications-Commercial Items, a completed copy of this provision shall be submitted with the offer unless ORCA registered which must be noted in quotation. 52.212-4, Contract Terms and Conditions-Commercial Items. 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, to include clauses: 5,14,15,16,17,18,19, 20, 23, 24, 31, and 32. The following additional clauses apply: CCR Clause: Attention: You must be registered in CCR to be considered for award. FAR 52.204-7, Central Contractor Registration (Oct 2003) (B)(2) the offeror shall provide its DUNS or, if applicable, it?s DUNS+4 number with its offer, which will be used by the contracting officer to verify that the offeror is registered in the CCR database. (C) Lack of registration in the CCR database will make an offeror ineligible for award.
