Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation, RMK0L070004, is issued as a Request for Quotes (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition 2005-17. The NAICS code is 335221 and the business size maximum is $5.0 Million. The proposed contract is a small business set-aside. All responsible small business sources may submit a quote, which will be considered by the Government. The contract shall be awarded as a firm fixed price contract. This office is requesting quotes on the following items: If you are interested in making a quote, please contact Judith Mariano at (505) 563-3491 and she can fax you a list of items needed. The location is: Office of Justice Services, Division of Corrections, Lower Brule Agency, 100 Akicita O? Tipi, Lower Brule, South Dakota. The QUANITY will be included in the faxed listing of items. Please, provide a breakdown of your costs. The breakdown shall include the delivery or shipping charges. Also, offeror should indicate how long it would take to have the equipment repaired or replaced if it should breakdown within the warranty period. The contract shall be awarded as a firm fixed price contract. The SOW is as follows: Background: The Bureau of Indian Affairs, Office of Justice Services, Division of Corrections, serves the new Lower Brule Justice Center Correctional Facility. This Facility is new and requires initial kitchen start up items for Phase I of the facility. OBJECTIVES: To have all items delivered within a three to four week timeframe and to setup any equipment as needed. RESPONSIBILITIES OF ALL RELATED SERVICE PROVIDERS: The service provider is to deliver all items requested within the stated timeframe and to have all equipment in good working condition. Please include your TAX ID Number, CAGE Number (obtain at www .ccr.gov), and DUNS Number when submitting your quote. The Offeror shall provide the sum cost with a breakdown of their cost to provide this service and respond to the Evaluation Factors delineated at the end of this synopsis. This solicitation incorporates one or more solicitation provisions by reference with the same force and effect as if they were given if full text. The Evaluation Factors are in descending order of importance. All evaluation factors other than cost or price, when combined are significantly more important than cost or price. The Evaluation Factors are: 1) Delivery within three to four weeks; 2) Price Reasonableness; 3) Warranties on all equipment; 4) The length of down time if equipment should breakdown. 5) Past Performance: Award will be based on the Best Value to the Government, overall acceptable quote, as submitted by a responsible offeror and ability to provide the above requirements. Quotes will be submitted in writing and signed by an individual with the authority to bind their respective company; electronic submissions via fax are acceptable.
