- This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Request for Quote is W9124B-07-T-0040 issued as request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Ac quisition Circular 2005-16 dated 22 March 07. The associated NAICS code is 326192. Contractor shall provide the following specific tasks: ITEM DESCRIPTION AND QUANTITY FOR FIRM FIXED PRICE (FFP) CLIN001: Remove & dispose of existing sheet vinyl flooring. Prepare floor for installation of Armstrong Medintech sheet vinyl. Furnish and ins tall Armstrong Medintech sheet vinyl in Freesia Pearl, #88485, in three (3) operating rooms that are approximately 20' x 21'. All three (3) operating rooms must have the sheet vinyl extended six (6) inches up the wall. All seams will be heat welded. Al l work is to be performed during normal business hours. The contractor shall furnish all materials, supplies, labor, and equipment to perform the requirement. Contractor, upon arrival, report to Joyce Johnson in Bldg 166, Room 463. The customer shall verify that materials to be disposed of are not contaminated. All equipment to be removed and replaced by the customer as needed. Requested Period of Performance: 26 29 Dec 2007 or otherwise determine by the customer.
