THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN SUBPART 12.6 AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION. PROPOSALS ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. THERE IS CURRENTLY NO FUNDING FOR THIS REQUIREMENT. ***************************************************************** Purchase Request Number: F3W3267214A002 - Request for Quotation This document incorporates provisions and clauses in effect through the Federal Acquisition Circular 2005-07. ***************************************************************** Line Item 0001: Aerobic Instruction Classes; Min 5 Classes, Up to 15 classes per week; Max 550 classes per year; Quantity: 550 Each Line Item 0002: Indoor Cycling Instruction Classes; Up to 4 Classes per week; Max 200 Classes per year; Quantity: 200 Each *** Period Of Performance: 1 OCT 07 ? 30 SEP 08 *** STATEMENT OF WORK GENERAL The Group Exercise Contract Manger (GECM) will provide the expertise needed to operate a well-rounded exercise program (i.e., step, floor, etc.) for the Spangdahlem, AB Fitness and Sports Center as defined in this statement of work. Classes are to be performed at the Skelton Memorial Fitness Center, Spangdahlem AB, Germany and the Bitburg Annex, Bitburg Germany. SCOPE OF WORK 1. The contractor will provide personnel transportation, music, microphones, materials, supervision and all other items necessary to perform aerobic classes as defined in the SOW. A minimum of five classes will be provided weekly at the Skelton Memorial Fitness Center, Spangdahlem AB. Classes will be offered Monday through Friday. Class times may vary at the discretion of the Fitness Center Director. Classes will not be held on U.S. holidays, but will operate at Fitness Center Director?s discretion on German holidays, down days and family days. 2. Contractor will provide a minimum of five aerobic classes per week and no more than 15 classes per week without Fitness Center Director?s written approval. 3. Contractor must provide current certified instructors in hi/lo impact, cardio-kick boxing, spinning, circuit training, ABS class, yoga or group strength training, etc. 4. National accreditation through an approved outside agency which assures the certifying organization has a standard by which exams are developed and monitored, and have current standards of practice within the discipline. Contractor and all his/her instructors must be currently CPR qualified and provide proof of certification to the Fitness Center Director prior to teaching aerobics classes. 5. Instructors must be certified through ACSM, American Council on Exercise (ACE) and the Aerobics and Fitness Association of America (AFAA). Other nationally recognized certifying organizations may apply to HQ AFSVS/SVPAF for equivalent recognition. The contractor will provide copies of all instructors? certifications to Fitness Center Director at start of contract or when new instructor is employed. SPECIAL EVENTS Contractor will be required to provide instructors for special events with 14 days notice or more. CONTRACTOR?S RESPONSIBILITIES 1. The contractor must provide the number and type of instructors for each aerobics class as specified in the contract. The contractor will provide the Fitness Center with a list of currently certified instructors and update the list as needed. 2. The contractor ensures assigned instructors arrive at site of the class at least 15 minutes prior to the start of aerobics class and 30 minutes prior to start of special events. 3. The contractor will sign-out aerobics room and equipment key from Fitness Center staff prior to class, secure equipment storage areas and room and return keys to fitness staff and the end of each scheduled class. 4. When passes are required for non-government instructors, the contractor provides the appropriate point of contact (POC) names, passport and social security numbers no less than eight days prior to the scheduled event to the 52d Services Squadron Command Section so authorizations can be obtained. 5. The contractor ensures incidents are reported verbally to the Fitness Center Director, or designated representative, by 1200 and a written report by 1600 the first duty day following the incident. 6. The contractor inspects the fitness area ensuring it is properly prepared (from a safety position) and all potential safety hazards were removed prior to any use of the fitness area. Contractor will be responsible for notifying Fitness Center Director in any case of equipment that is broke, unsafe or unusable at the end of each class so that the aerobic room can continue to meet the needs of instructors and patrons. The contractor will turn on/off room lights, fans, electronic equipment and transformers. The instructor will complete the class roster for each scheduled class to include instructor?s name, time of class and names of students in class. This acts as verification of classes conducted, if roster is not complete at the end of each class payment will not be made. 7. The contractor ensures all required paperwork is provided to the Fitness Manager?s office by 0800 the first duty day after classes are completed. This includes providing attendance sheets with proper information recorded on the documents for the record keeping purposes. Ensure all instructors? sign in with name, rank, squadron and duty phone for each class. 8. The contractor must notify the Fitness Center as soon as possible if for unforeseen reason the instructors will not show up for scheduled classes. Contractor must notify Fitness Center Director and/or Fitness Manager, if they are unavailable contractor needs to leave a message and follow up by e-mail. If the contractor fails to notify the Fitness Center they will be charged monetary penalties equal to that days instructors pay. GOVERNMENT RESPONSIBILITIES 1. The Fitness Center Director will provide contractor with class schedule by the 15th of each month. The contractor will return the revised calendar by the 20th of each month. Fitness Center Director will also assist contractor when base passes for all non-government officials are necessary. 2. Provide locker room and shower facilities for aerobic instructors (daily use lockers are available). Provide the contractor with access to aerobic/spinning rooms for practice sessions. 3. Ensure all equipment provided to contractor is in working condition prior to aerobics classes/special events. 4. Provide the use of copier, DSN/commercial line and fax machine needed by contractor for administrative purposes. 5. When base emergencies, mission commitments, deployments and base closures happen, the contractor will be paid for all classes scheduled that day. The Fitness Center will give the contractor 48 hours notice when events will resume again. 6. Invite contractor/project manager/representative to attend the monthly/quarterly fitness and sports/HAWC council meetings as a nonvoting member. ACCESS TO FACILITIES The POC ensures all assigned instructors have access to all facilities at scheduled events.
