Description

Contractor to fill first time prescriptions for Department of Veterans Affairs patients seen at the Dubuque Outpatient Clinic, Mercy Health Center, 250 Mercy Drive, Dubuque, IA, to provide medication counseling per OBRA 90 requirements to adhere to the VA formulator and to provide prescription pads for the writing of these prescriptions for the period of one year starting 10/1/07 (date of award). Two option years are included. This requirement is 100% set aside for Service Disabled Veteran Owned Small Business. The NAICS code is 446110 Standard Size $6.5 Million. Contractor must be located within (1-3) miles driving distance of the clinic. The procurement shall be conducted in accordance with FAR 13 Simplified Acquisition Procedures.
