- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The reference number for this procurement is F2C1EC7212A001 and is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-18, Defense Federal Acquisition Regulation Supplement Change Notice (DCN) 2007-0802 and Air Force Acquisition Circular (AFAC) 2007-0531. For informational purposes the North American Industry Classification System code is 621111, with a small business size standard of $9M. This acquisition will be awarded as a firm fixed priced contract. Contract will consist of five (5) line items. Delivery and acceptance will be FOB destination for services; Hickam AFB, Hawaii 96853. CLIN 0001 Medical Coordinator services for FY ?08 (1 October 2007 through 30 September 2008). Quantity: 12 Months CLIN 1001 Medical Coordinator services for FY ?09 (1 October 2008 through 30 September 2009). Quantity: 12 Months CLIN 2001 Medical Coordinator services for FY ?10 (1 October 2009 through 30 September 2010). Quantity: 12 Months CLIN 3001 Medical Coordinator services for FY ?11 (1 October 2010 through 30 September 2011). Quantity: 12 Months CLIN 4001 Medical Coordinator services for FY ?12 (1 October 2011 through 30 September 2012). Quantity: 12 Months Quotes should be in the following format: Price for each month independently and total price for the year. This requirement is for the item(s) and services described above and in the SOW. Award will be made to the offeror whose offer conforming to the solicitation is determined to be the most advantageous to the Government. Award will be made to the lowest evaluated price among quotes meeting (or exceeding) technical acceptability. Offeror must ensure the provision FAR 52.212-3 (Alt I), Offeror Representation and Certifications-Commercial Items (this must be completed on http://orca.bpn.gov), DFARS 252.212-7000, Offeror Representation and Certifications-Commercial Items, and DFARS 252.225-7000, Buy American Act-Balance Payment Program Certificate (Jun 2005) is included with the quotation. Clauses and provisions may be accessed via the Internet, https://farsite.hill.af.mil or http://www.arnet.far.gov. The following clauses and provisions are incorporated and remain in full force in any resultant purchase order. FAR 52.212-1, Instruction to Offerors-Commercial Items, applies to this acquisition. The following additional clauses apply to this acquisition: FAR 52.217-8, Option to Extend Services (with at least 30 days prior to contract expiration as the fill-in); and 52.217-9, Option to Extend the term of the Contract (with at least 30 days prior to the contract expiration and at least 60 days before the contract expires as the two fill-ins for paragraph ?a? and shall not exceed 60 months as the fill-in for paragraph ?c?). ALL PROPOSALS MUST REACH THIS OFFICE NO LATER THAN 4:00 PM, Hawaii Standard Time (HST), on 19 September 2007.
