- THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN FAR SUBPART 12.6-STREAMLINED PROCEDURES FOR EVALUATION AND SOLICITATION FOR COMMERCIAL ITEMS-AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; QUOTATIONS ARE BEING REQUESTED, AND A WRITTEN SOLICITATION DOCUMENT WILL NOT BE ISSUED. This is a solicitation of Purchase Request F1W0HC7226A002 and is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-17. The associated North American Industrial Classification System (NAICS) code for this procurement is 813110 with a small business size standard of $6,500,000.00. This acquisition is set aside for small businesses. All responsible Contractors shall provide a quote for the following: Line Item 0001: Youth Ministry Director, 12 months STATEMENT OF WORK 1. SCOPE OF WORK 1.1 The Youth Ministry Director (YMD) shall provide all labor, equipment, materials, and transportation necessary to perform this contract. The YMD shall provide and coordinate chapel and chapel related youth ministry for junior and senior high school students. Many junior and senior high military family members attend schools in the local area. The YMD shall coordinate with local schools, churches, and youth agencies to ensure overall coordination of Elmendorf youth ministry. The YMD shall coordinate all Elmendorf AFB youth events with the 3 SVS Family Support Flight Chief. All program conflicts that cannot be resolved directly shall be communicated to the Wing Chaplain and 3 SVS/CC for resolution. Services rendered by the YMD shall consist of providing a base wide religious program, and supporting specific youth ministry needs in addition to those provided by the Elmendorf Chapel Team or volunteers, organizations, and agencies. The major scope of work required shall focus on conducting activities for the military dependent youth associated with the Elmendorf AFB community and to support the established programmed services of the Elmendorf Chapel Ministry, to include programming, developing and implementing youth activities and general Christian Religious Education. 1.2 Responsibilities. The following program and support requirements are considered minimum elements of ministry to junior and senior high youth: Responsibilities are based on a 20-hour workweek. - Develop supportive relationships with youth, maintain close liaison with chaplains, command and local school districts. (ref. 1.2.11) - Organize and direct regular junior and senior high youth outreach club meetings. - Organize and conduct weekly Bible study or small group activities for junior and senior high youth. - Provide an ecumenical youth outreach ministry of presence wherever youth gather within the base or local community (e.g., school activities, sports events, recreational centers, etc.) - Conduct one (1) or more youth retreats or service projects each year. - Conduct twenty (40) youth outreach events per year emphasizing relationship building, social interaction, and recreation. - Refer individuals to appropriate helping agencies as necessary (e.g., Life Skills, Family Support, Family Advocacy, etc.) - Help organize an adult support committee to suggest, plan, support, evaluate, and implement youth programs. (ref. 1.2.10) - Recruit and train adult volunteers to assist in the ministry. - Submit reports to the Wing Chaplain outlining and evaluating ministry activities and plans as directed. (ref. 1.2.2) - Develop supportive relationships with other community agencies (e.g., Services, Teen Center, Outdoor Recreation, etc.) 2. PERSONNEL 2.1 The YMD shall be an active member of the Elmendorf Chapel Team and shall be required to attend all mandatory staff meetings and functions. In the event of a temporary absence, the YMD shall provide adequate coordination (15 days notice) and identify to the Wing Chaplain the individual assuming YMD duties. 2.2 Program shall be Christian nondenominational in programming and ecumenical in nature. No faith, ethnicity, gender, race, or denomination shall be denigrated in any way. 2.3 The YMD shall be extensively trained in youth work. Related baccalaureate level academic credentials are required. Seminary education is desirable. 2.4 The YMD shall have a minimum of two years successful full time experience in youth ministry. References from all previous ministry experience shall be required. 2.5 As cited above, the YMD shall be qualified in youth ministry as a result of previous training, education and experience. The YMD is expected to maintain current skills by procuring and attending ongoing upgrade training on at least an annual basis. This shall be done at opportune times so as to minimize any potential conflict with existing youth ministry requirements. 2.6 The YMD shall exercise discretion regarding persons counseled or interviewed, and regarding any other information relating to individuals participating in the Elmendorf Youth Ministry Program. The YMD shall not relate or provide sensitive information regarding such individuals, nor shall the YMD provide originals or copies of personal notes, checklists, or counseling records regarding such individuals to any other persons without express approval from the Supervisory Chaplain, Wing Chaplain, or other competent authority. This duty of discretion shall in no way be construed as providing the YMD the power or ?priest-penitent? privilege as provided in law. 2.7 In accordance with Dept. of Defense Instruction (DODI) 1402.5, each individual working with children under the age of 18 in conjunction with this contract must successfully complete a federally mandated criminal history background check (conducted by the Base Chapel). Failure to satisfactorily complete the criminal history background check shall result in termination of the contract. 3. SUPPORT IN KIND 3.1 3 WG/HC will provide the following non-monetary support in kind to facilitate the accomplishment of ministry by the YMD. - Office space and base and local telephone access. - Facility to hold activity/Religious Education meetings as part of the ministry. - Limited access to government vehicles (i.e. buses and/or vans) in support of specific ministry events such as excursions and retreats. Transportation requirements shall be approved by 3 TRANS and coordinated by 3 WG/HC. 4. QUALITY ASSURANCE 4.1 The Wing Chaplain or designee will maintain periodic on-site liaison and frequent telephone and written communication with the YMD. Such liaison will identify potential or actual problem areas in fulfilling the requirements of the contract. All activities shall be clearly documented and reports shall be filed with the Wing Chaplain to include a proposed events report. 4.2 Any discrepancies or dissatisfaction in the fulfillment of the contract shall be expeditiously corrected by revised programming, as required. This most likely would result from direct liaison with the supervisor or wing chaplain. 4.3 The Elmendorf YMD?s POCs concerning this program will be the Supervisor Chaplain for all matters pertaining to youth ministry and the Wing Chaplain for matters involving training and overall policy. Communication shall be in person, by telephone, fax or email. 4.4 The YMD shall establish and maintain a Quality Control (QC) plan acceptable to the Wing Chaplain. The QC plan shall include a method of identifying deficiencies in the quality of services performed before the level of performance becomes unacceptable. The QC plan shall establish procedures for corrective/preventive action. Documentation of all YMD quality control reports and corrective actions will be maintained by the Wing Chaplin. 4.5 The YMD shall contact the Contracting Officer, 3rd Contracting Squadron/LGCB to address contractual matters. 5. KEY CONTROL AND SECURITY 5.1 The chapel will supply building keys necessary to perform all programs. The YMD shall be responsible for keys provided by the chapel. Keys shall be safeguarded and shall not be duplicated. 5.2 The YMD shall report any, lost or misplaced keys to the chapel facilities manager within one hour of discovery of the occurrence. 5.3 The YMD shall be responsible for security of facilities and any property made available by the chapel. 6. SAFETY 6.1 The YMD shall implement safety practices in accordance with Base and Chapel Air Force Instructions (AFIs) and Operating Instructions (OIs). 6.2 The YMD shall perform preventative maintenance checks on all vehicles prior to vehicle operation in accordance with (IAW) 3 TRANS OIs. 6.3 As part of the chapel team, the YMD shall enroll in base safety related training (e.g., SABC, CPR, etc.) 7. FURNISHED PROPERTY AND SUPPLIES 7.1 In addition to paragraph 3, Support in Kind, the following equipment may be provided: Use of an overhead projector, television and VCR, use of copy machine to include lawful reproduction privileges, use of videos for teaching and programming as available in Chapel Resource Center. 7.2 Class and meeting rooms with chairs, teacher?s desk, chalk or white boards, pencil sharpener, etc. for performance of this contract. These properties will not be assigned permanently to the YMD as they are used for other events. 7.3 Additional Supplies. YMD shall furnish all property and services not specifically identified in 1.8 Furnished Property and Supplies and 1.4 Support in Kind, but are necessary to perform the requirements of this contract. Service start date shall be no later than 28 September 2007.
