- This is a combined synopsis/solicitation for commercial items prepared in accordance with the Federal Acquisition Regulation (FAR) subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the on ly solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number for this Request for Quotation is W51XTP72264647. The solicitation document and incorporated provisions and clauses are those in effect thr ough Federal Acquisition Circular 2001-13. This solicitation is 100% set aside for small business; the associated NAICS code is 334510, no. of employees: 500. This requirement is for the GI Clinic, Evans Army Community Hospital, Fort Carson, CO. Federal Acquisition Regulation (FAR) Clause 52.211-6 Brand Name or Equal applies. This requirement is for Small Bowel Capsule Item#20451, Given Imaging or equal, 24 each. Item no. 0001: Small Bowel Capsule Item#20451 Given Imaging or equal, 24 each. Price shall include, 240 each, Patient Information English version, at no charge. VENDORS OFFERING OTHER THAN THE BRAND NAME PRODUCT SHALL MEET OR EXCEED THE FOLLOWING SALIENT CHARACTERISTICS: Item no. 0001: Small Bowel Capsule Item#20451 Given Imaging or equal, 24 each. Wireless capsule measuring 11 x 26 mm (about the size of a large vitamin) and has a coating on it that makes it easy to swallow. Capsules must be approved by the Food and Drug Administration. Capsules shall contain a color video camera, a wireless transmitter, 4 lights, and a battery. Capsules shall be able to capture more than 57,000 crystal clear images of the small intestine in eight hours. Required Delivery: Within 30 days of contract award to Evans Army Community Hospital, Fort Carson, CO 80913-5000 FOB: Destination. Vendors shall comply with FAR 52.211-6 if submitting other than the brand name products.
