- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Request for Quote (RFQ) VA-256-08-RQ-0019 is assigned to this solicitation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-20. NAICS 811219 with a size standard of $6.5 million. This will be a sole-source procurement for equipment located in the Operative Care Line at the Michael E. DeBakey VA Medical Center, Houston, TX. Only one vendor, Intuitive Surgical, is known to offer preventive and corrective maintenance services on DaVinci Surgical System, thus the services will be procured through full and open competition. Applicable lines items: 1. Contractor to provide all labor, travel, and material (exclusive of instruments and accessories) for preventive maintenance in accordance with manufacturer's recommendations. Services include guaranteed response time within 2 hours of notification and software enhancements for existing features. Services shall be provided Monday through Friday between the hours of 8:00 a.m. - 4:30 p.m. exclusive of federal holidays. Contract period is November 1, 2007- October 31, 2008 with one 1-year option. The provision at 52.212.-1 Instructions to Offerors - Commercial applies to this acquisition. The quotation is to be received in the Contracting Office, Room 4B-307, 2002 Holcombe Blvd., Houston, TX 77030-4298 no later than 11:00 a.m. October 26, 2007. The quotation will be evaluated on the basis of price and past performance which includes contractor's proven record to maintain specified system. Offerors' response must include a completed copy of the provision at 52.212-3 Offeror Representations and Certifications - Commercial Items. This provision may be downloaded at http://www.acqnet.gov/far/. Offeror must include the certification at 852.219-70 Veteran-Owned Small Business.
