Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation shall not be issued. Solicitation VA-259-08-RQ-0011 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-19. This is a competitive procurement under the North American Industrial Classification (NAIC) Code 562111. The Eastern Colorado Health Care System (ECHCS) intends to contract for general waste pickup and removal for the Fisher House throughout the year, with the possibility of four (4) option years. The Services to provided for Fisher House, 1954 Quentin Street, Aurora, CO 80010 shall be: The rental/lease of a four-yard roll off dumpster to be used for construction waste as well as other general waste. Fifty-two (52) pick-ups will be needed throughout the year on a consistently established day of the week, Monday through Friday between 8:00am and 4:00pm. The four-yard roll off dumpster will be located at the east end of the Fisher House parking lot and replaced/cleaned at least quarterly. The rental/lease of a four-yard roll off single stream recycle dumpster, to be used for recycle of newspaper, office paper, cardboard, tin can, aluminum, glass, and plastic. Fifty-two (52) pick-ups will be needed throughout the year on a consistently established day of the week, Monday through Friday between 8:00am and 4:00pm. The four-yard roll off recycle dumpster will be located at the east end of the Fisher House parking lot and replaced/cleaned at least quarterly. In accordance with FAR 52.212-2, the Government shall award a contract resulting from this solicitation to the responsible Vendor whose offer conforming to the solicitation shall be most advantageous to the Government, price and other factors considered. Proposals shall be evaluated on the basis of Technical and Cost Factors. Award shall be made on the basis of the lowest evaluated price of proposals meeting or exceeding the acceptability standards for non-cost factors. The Vendor shall submit adequate information for evaluation purposes on the following evaluation criteria. Each are shall be specifically labeled and identifiably separate from other areas. Price shall be submitted under separate cover from the other evaluation criteria. Past performance information is one indicator of a Vendor's ability to perform the contract successfully. The currency and relevance of the information, source of the information, context of the data, and general trends in Vendor's performance shall be considered. Provide a list of your contracts similar to this proposed contract. Vendors shall submit the following information: Name and address of the agency or firm with whom the contract is/was held; Name and phone number of responsible contracting official administrating the contract; Contract number, dollar amount and date of award or period of performance; and Description of the contract and status (i.e. in progress, completed, etc). The Vendor's capabilities to perform trash removal as described herein should be described as follows: dumpster requirements; recycling requirements; schedule requirements; Ability to meet all licensing/permitting, insurance and any other requirements as required by federal, state and local regulations. The Government shall evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). A written notice of award or acceptance of an offer mailed or otherwise furnished to the successful Vendor within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. The provisions at FAR 52.212-1, Instructions to Offerors-Commercial items applies to this acquisition with the following changes, items (e) and (h) of the standard clause are deleted. Offerors shall include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial items, with their offer. The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition which shall include the following clauses: 52.219-14, 52.222-19, 52.222-21, 52.222-35, 52.222-36, 52.222-37, 52.222-41, 52.222-42, 52.225-3, and 52.232-33. The following standard FAR and VAAR clauses are incorporated and apply to this acquisition: FAR 52.203-3, 52.203-7, 52.203-10, 52.215-6, 52.215-8, 52.215-19, 52.217-8, 52.217-9 (which provides that the Government may extend the term of the contract within 60 calendar days, provided that the Government shall provide a preliminary notice of intent at least 60 calendar prior to the expiration date; and services under this contract shall not exceed five (5) years), 52.232-18, 52.232-19 (which provides that funds are not authorized past September 30 of any fiscal year, unless written authorization is received from the Contracting Officer), and VAAR 852.270-1. Interested offerors may submit their proposals along with a completed copy of the provision at FAR 52.212-3 Offerors Representations and Certifications-Commercial Items, on or before 4:30 p.m., October 24, 2007.
