Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and A WRITTEN SOLICITATION WILL NOT BE ISSUED. The solicitation number for this procurement is FA4830-08-Q-0002 and is issued as Request for Quotation. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-16, Defense Acquisition Circular (DAC) 91-13 correction, DFARS Change Notice (DCN) 20070212, and Air Force Acquisition Circular (AFAC) 2007-0314. This solicitation is 100% set aside for small business IAW FAR 19.502-2. The North American Industry Classification System is 711510. The BID SCHEDULE shall be for: LINE ITEM 0001 QTY 1 EA: Wingman Day Speaking Services (see Statement of Work for requirements - price must include all associated cost to include travel and lodging) DELIVERY: Shall be delivered to MOODY AFB GA 31699. FOB POINT: Destination price must include shipping. Provision at FAR 52.212-1, Instruction to Offerors Commercial Items applies to this acquisition. The following addenda is provided to this provision: Request for Quotation should be submitted and shall contain the following information: SOLICITATION NUMBER; TIME SPECIFIED FOR RECEIPT OF OFFERS; NAME; ADDRESS; TELEPHONE NUMBER OF OFFEROR; TERMS OF THE EXPRESSED WARRANTY; PRICE, ANY DISCOUNT TERMS AND ACKNOWLEDGEMENT OF ALL SOLICITATION AMENDMENTS. Quote shall also contain all other documentation specified herein. Signed quotes must indicate quantity, unit price and total amount. Offerors that fail to furnish the required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration. EVALUATION/AWARD: IAW FAR 52.212-2, Evaluation-Commercial Items, the Government will award a purchase order resulting from this solicitation to the Best Value offer. All offerors MUST BE CENTRAL CONTRACTOR REGISTERED (CCR). The point of contact for this solicitation is Lt Davis, (229) 257- 4917 (phone), (229) 257-4032 (fax), or william.davis@moody.af.mil STATEMENT OF WORK (SOW) 23rd Wing SCOPE: Wingman Day support as a "Living Life at High Performance" speaker for all units to strengthen the wingman concept and personal decision points in the lives of airmen in the 23rd Wing. The Culture of Responsible Choices (CoRC) is a commanders program and consists of a four-tiered approach with emphasis on leadership, individual, base, and community-level involvement -- underscoring responsible behaviors. 1. The contractor is responsible for: The empowerment of individuals and organizations to be more productive, operate more in concert with personal and organizational values and beliefs, and be character-centered in daily life is our goal. Desired Training topics should include: - Establish a culture of responsible choices - Empower ethical leadership - Promote four dimensions of wellness: -Emotional, Social, Spiritual, Physical - Encourage Airmen to act and drink responsibly - Bring to the forefront real issues faced by the USAF: - Suicide prevention - Family separation - Sexual Assault - Promote positive transformation - Reduce the stigmata of seeking help - Enhance family support and relationships Participants should take away concepts from the training that include : - being empowered with new leadership skills - discovering their own success strategies - be energized and inspired in the workplace and beyond - be encouraged to take risks in their thoughts and actions - understand the value of collaboration and teamwork - be entertained and educated in a fun and humorous way Overall topics will be included to help participants understand the principles of: - Being accountable - Taking responsibility for your life - Balancing priorities - Thinking before you act - Leadership - Everyday is a new opportunity - Your attitude dictates your potential future The following will be required: Four 1 - 1.5 hour speaker presentations for a maximum of 2000 participants per session covering the goals and desired outcomes listed above. The goal is a kick-off event for the newly established Community of Responsible Choices Program newly launched by the Air Force. There will be four 1 - 1.5 hour seminar session conducted on 7 December 2007 for the 23rd Wing at 0730 -1000 -1200- 1400. The government maintains the right to conduct less than four sessions on 7 Dec if necessary and move the times as necessary to meet flying and maintenance schedules. The contractor must be available 0730 -1530 on 7 Dec at the discretion of the 23rd Wing. The last session on 7 Dec must be complete by 1530. 2. MATERIALS: Wallet cards 3x2 in color printed on card stock with main points of the speaker should be included with the program to include a leadership quote in a readable font from the 23rd Wing commander as appropriate per session. The 23rd Wing logo will be provided by Lt Will Davis, 229-257-4917. 2000 sets of juggling balls will be provided by the speaker for the 23rd Wing Wingman seminars on 7 Dec 2007. 3. LOCATION: The sessions will be conducted at the Fuels Barn, B649. 4. WORKMANSHIP: The contractor will conduct a live speaking performance at all sessions. Slide shows or other performance props or equipment will be optional for the contractor.
