- Amendment Number ONE. THE FOLLOWING TEXT REPLACES THE ORIGINAL SYNOPSIS/SOLICITATION. The text in ALL-CAPS under Line Item 0001 was added to the original requirement description. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the ONLY solicitation; a written solicitation will not be issued. The reference number is F2FT137334A001. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-21, Effective 7 Nov 2007 & Class Deviation 2005-o0001. This award is unrestricted and is under North American Industry Classification System Code 541690 and the small business size standard is $6.5 M. The requirement consists of the following: The 1st Special Operations Contracting Squadron at Hurlburt Field FL intends to award a purchase order to The University of Alabama at Birmingham, for Line Item 0001: Commercially acquire 25 Immature Yorkshire swine weighing approximately 35-70 kg and provide surgical skills training on these animals, one animal will be used per training, two training sessions per month should be scheduled; making a requirement of 24 animals per year, one extra animal is requested to cover unforseen health issues. A Veterinarian and assistant veterinarian should be on site at the time of the procedure, the animals should be sedated prior to the procedure using 15-30 mg/kg Ketamine & 0.11-0.22 mg/kg Acepromazine, and 2-3% Isofluorane in 60% oxygen or 30-200 mcg/kg/hr Fentanyl & 2-8 mg/kg Ketamine in order to maintain sedation during procedure. All animals must be euthanized at the completion of the surgical training. IN ADDITION TO THE ABOVE MENTIONED REQUIREMENT AFSOC PARARESCUEMEN MUST BE TRAINED ON EMERGENCY MEDICINE, AND TRAUMA. THE INSTITUTION PROVIDING THE EMERGENCY MEDICINE, SURGERY AND TRAUMA MUST BE ACCREDITED BY THE JOINT COMMISSION ON ACCREDITATION OF HEALTHCARE ORGANIZATIONS (JCAHO). TRAINEES MUST BE ALLOWED TO PRACTICE IN A HOSPITAL EMERGENCY ROOM ENVIRONMENT, AND WITH HUMAN ER PATIENTS. PROFESSIONAL LIABILITY (MALPRACTICE) COVERAGE, IN AMOUNTS THAT ARE REASONABLE AND CUSTOMARY IN THE COMMUNITY FOR THE APPROPRIATE SPECIALTY, COVERING LIABILITY FOR PERSONAL INJURY OR PROPERTY DAMAGE, INCLUDING LEGAL REPRESENTATION AND EXPENSE OF DEFENSE OF ANY SUCH LIABILITY CLAIMS, ACTIONS OR LITIGATIONS, RESULTING FROM PARTICIPATION BY AIR FORCE TRAINEES UNDER THE CONTRACT MUST BE PROVIDED. Items are to be delivered FOB destination for inspection and acceptance by an authorized government official.
