Description
Section SF 1449 - CONTINUATION SHEET STATEMENT OF WORK STATEMENT OF WORK 1. The contractor shall provide all equipment, material, labor, supervision, supplies and items necessary to support the Washington Army Nat ional Guards Drill Training to be conducted at Yakima Training Center (YTC), Yakima Washington. 2. The primary locations for the preparation and serving of two breakfast meals and one dinner meal will be 2 fixed dining facilities at YTC, two for the weekend of 5 and 6 January 2008 and one for the weekend of 12 and 13 January 2008. The contractor will be required to provide their own labor, supplies and subsistence to prepare and serve the above mentioned meals. 3. The numbers in this soli citation are estimates. The firm number will be provided to the awarded contractor five business days prior to performance by Mr. Frank Gibson. 4. The breakfast meal will be ready to serve between the hours of 6:00 a.m. and 7:30 a.m. The dinner m eal will be ready to serve between the hours of 5:00 p.m. and 6:30 p.m. 4. The contractor shall provide condiments, disposable plates, disposable utensils and napkins (disposable). 5. The contractor is responsible for the cleanliness of the dinin g facilities in accordance with TB Med 530. 6. Food supplies must be purchased from approved sources that meet the U.S. Food Code and the TB Med 530. 7. The contractor must meet the requirements set by Madigan Army Medical Center Environm ental health to feed on a Federal Installation. The phone number for MAMC Environmental Health is (253) 968-4113. This agency will provide all requirements needed to meet standards to feed on the installation. 8. The contractor shall adhere to the feeding and storage requirements of TB MED 530 chapters 4, 5, 7, 8, 11, 12. The TB MED 530 can be found at the following web address: http://www.army.mil/usapa/med/DR_pubs/dr_a/pdf/tbmed530.pdf. The referenced chapters can be printed for reading. <B R>9. The menus for the meals will be provided at the site visit. 10. SITE VISIT INFORMATION Site visit will be conducted on 17 December 2007. 1000 hrs Prospective Contractors are highly encouraged to attend. Location: Yakima T raining Center Bldg 262 Yakima, WA 98901 For directions and access requirements to Yakima Training Center, contact Mr. Reddick at (509) 577-3201. Purpose of this visit is as follows: 1. Walk the site for set up of any required e quipment needed to meet the feeding and storage requirements. 2. Look at Dining Facilities. 3. To discuss the utility (water, electricity, sanitary sewer, telephone) arrangements required for the performance of this contract. 4. Any other coord inating issues regarding the contract line items. 5. To finalize dates for set up, performance and tear down.
