- This is a combined synopsis/solicitation for commercial items prepared in accordance with (IAW) the format in FAR Subpart 12.6, as supplemented with additional information included in this notice in conjunction with the procedures in FAR Part 13. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation VA258-08-RQ-0045 is issued as a request for quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-19. No telephone requests will be accepted. Only written requests received directly from the requester are acceptable for additional information. If requesting by FAX, you may send your request to 602-212-2163, ATTN: Enrique Rivera. The North American Industry Classification System (NAICS) number is 339113 and the business size standard is 500. REQUESTERS MUST STATE THEIR BUSINESS SIZE ON EACH REQUEST. Service disabled veterans owned small business are encouraged to submit offers. This requirement is set aside for small business. This requirement is being solicited as a brand name or equal purchase. The line item listed below is manufactured by OMEGA TRACER. The line item is as follows: Line Item 0001 ??? 1 each Omega Carver, or equal; to include Omega Tracer Home-base software, security key, 2-year manufacturer???s warranty, 2-year extended mfr???s warranty, and 3-year technical support. FOB destination delivery date of January 31, 2007 to Warehouse- Department of Veterans Affairs, VA Medical Center, 650 E. Indian School Rd. Phoenix, AZ 85012. Offerors must comply with all instructions contained in provision 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. The following additional provisions and clauses apply: 52.211-6, Brand Name or Equal, 52.212-2, Evaluation-Commercial Item, the Government will award a firm-fixed price contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the government, considering price and price related factors. 52.212-3, Offeror Representations and Certifications-Commercial Items, a completed copy of this provision shall be submitted with the offer. 52.212-4, Contract Terms and Conditions-Commercial Items. 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, to include clauses: 15,16,17,18,20,25,27, and 33. 52.233-2, Service of Protest.
