Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation number VA-244-08-RQ-0160 is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-24. The North American Industry Classification System (NAICS) number is 337214 and the business size standard is 500. This requirement is 100% set aside for small businesses. REQUESTERS MUST STATE THEIR BUSINESS SIZE ON EACH QUOTATION. The Department of Veterans Affairs Medical Center, 1111 East End Blvd, Wilkes-Barre, PA 18711, intends to procure medical recliners and accessories particular to Scope MD. The VAMC is accepting quotations for the following items: (1) 15 each, Scope MD medical treatment recliners, model number MD204, 450lb capacity, (2) 02 each, Scope MD bariatric recliners, model number HD1030, 650lb capacity, (3) 34 each, side tables, (4) 17 each, retractable pull-out foot trays. The color selection for all recliners will be Kilkenny Tweed Rosebud. Delivery shall be FOB Destination to the Department of Veterans Affairs Medical Center, 1111 East End Blvd, Wilkes-Barre, PA 18711 within six weeks after receipt of order. The VAMC has determined that these products are the only items that can meet their needs. The primary features are both arms swing out providing for safe patient transfer and easy access to patient vital areas, multiple position adjustments and the ability to go into a true Trendelenburg position and the upholstery can be easily removed and replaced if damaged. The provision at 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. Award will be made to the responsive, responsible offeror with the lowest evaluated price; consequently, the provision at 52.212-2, Evaluation-Commercial Item does not apply to this acquisition. Price will be evaluated to determine its fairness, completeness and reasonableness as it relates to the items offered. The following provisions and clauses apply: The offeror must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items. Clause 52.212-4, Contract Terms and Conditions-Commercial Items applies to this acquisition with the following provision/clauses added by Addenda: 852.270-4, Commercial Advertising. In addition, Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition and the following clauses as listed under 52.212-5 apply: 52.219-6, Notice of Total Small Business Set-Aside; 52.219-28, Post Award Small Business Program Re-representation; 52.222-19, Child Labor ??? Cooperation with Authorities and Remedies; 52.222-21, Prohibition of Segregated Facilities; 52-222-26, Equal Opportunity; 52.222-36, Affirmative Action for Workers with Disabilities; 52.222-50, Combating Trafficking in Persons; 52.225-3, Buy American Act ??? Free Trade Agreements ??? Israeli Trade Act; 52.225-13, Restrictions on Foreign Purchases; 52.232-34, Payment by Electronic Funds Transfer ??? Other than Central Contractor Registration. This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text. Upon request, the contracting officer will make their full text available. Your company must be actively registered in CCR to be considered for award. Quotations must include the RFQ number, the proposed unit price, the proposed extended price, the proposed delivery schedule, any prompt payment discount terms, company name, and point of contact, address, phone number, fax number, tax identification number, DUNS number, and size status (i.e., disabled-veteran-owned business, veteran owned business, disadvantaged business or woman owned business or small business).
