Description
EVALUATION FACTORS FOR AWARD The Technical Evaluation Factors for award are listed below. Technical and Past Performance are weighted equal, and when combined, they are equal in importance to Price. Please place your technical proposal in an envelope separate from your price proposal. Do not identify any pricing in your technical proposal. Please send only one (1) copy of the technical and price proposals. There is one GO / NO GO evaluation factor that will determine if your proposal will be considered for award. Offeror must provide the following: Proof of Insurance in the required amount as identified in the Statement of Work, copy of Hazardous Materials License, and proof of driver bonding. If this is not provided your proposal will not be considered for award. 1. Technical Evaluation Factor QUALITY ASSURANCE PLAN Offeror must provide documentation that shows what procedures will be utilized to track, identify, prevent, remedy and ensure non-recurrence of defective services and how customer complaints are tracked and resolved. Offeror???s proposal will be evaluated based on the strengths, weaknesses and deficiencies in this area. 2. Past Performance a. List of References Offeror shall submit at least three (3) prior contracts that are similar in size and scope to this requirement, include any Federal, State, local government and commercial contracts. The Government will consider offeror???s record for delivery, quality and past or present malpractice judgments, proposed major subcontractors as it relates to the probability of successfully performing the solicitation requirements. Past performance evaluations will be conducted using information obtained from Contractor Performance System (CPS) or Past Performance Retrieval System (PPIRS) and any other sources deemed appropriate. While the Government may elect to consider data obtained from other sources, the burden of providing current and complete past performance information rests with the offeror. The past performance information obtained will be used for both the responsibility determination and this evaluation factor. Information that is to be submitted is as follows: (a) Contractor name, address, phone and fax numbers (b) Description of requirement, contract number, value and award and completion date (c) Any problems associated with the contract and how you worked to resolve them. Offeror will be evaluated on customer service, timeliness of performance, quality of services provided. In the case of an Offeror without a record of relevant past performance or for whom information on past performance is not available, the Offeror may not be evaluated favorably or unfavorably on past performance. b. Number of Years in Business as a Courier Offeror with more years in business may receive a more favorable rating in this area. 3. Price
