- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is IHS-SOL-0800105 for Security Guard services on an as needed basis. This solicitation is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-24. This acquisition is 100% set-aside for small businesses. The NAIC code is 561612 and the small business size standard is $17M. This acquisition will be subject to Service Contract Act. Multiple awards are anticipated. Base Year is for the Period June 1, 2008 through May 31, 2009 Line Item 1 is Security Guard. Hourly rate $____ NTE 2080 hours/yr Option Year 1, for the period June 1, 2009 through May 31, 2010 Line Item 1 is Security Guard. Hourly rate $____ NTE 2080 hours/yr Option Year 2 is for the period June 1, 2010 through May 31, 2011 Line Item 1 is Security Guard. Hourly rate $____ NTE 2080 hours/yr Option Year 3 is for the period June 1, 2011 through May 31, 2012 Line Item 1 is Security Guard. Evaluation will be based on price, experience – Contractor must have CLEET certification and at least 1 year experience or past law enforcement experience and Past Performance - The Contractor shall provide past performance references with name, contact name, period of performance and telephone number. Contact Yolanda Reyna at yolanda.reyna@ihs.gov for information regarding the solicitation or fax number 918-342-6675. 1.DESCRIPTION OF CONTRACT: This is a non-personal service contract to provide Security Guard coverage for the Claremore Indian Hospital (CIH), 101 S. Moore Avenue, Claremore, Oklahoma 74017 beginning June 1, 2008 through May 31, 2009. The CIH is 102,000 sq. ft. with two detached buildings each @ 860 sq. ft and 300 sq. ft. with 50 bed general medical and surgical hospital. 2.SCOPE OF WORK: a)Provide internal building security by monitoring doors, locks and windows; examines and reports any tampered locks. b)Patrols building and grounds to protect property, hospital employees and visitors from the threat or danger of accidental injuries, fire, theft or intrusion. c)Checks any unauthorized or suspicious persons on premises; will properly route or detain them, according to circumstances. d)Restrains any belligerent or intoxicated visitor for proper handling and/or disposal. e)Immediately reports to Supervisor or respective evening or night charge nurse any fire, injury, safety hazard or unusual condition, malfunctioning of heating, cooling, plumbing or electrical system. f)Enforces the hospital regulations governing patient visitation hour. g)Enforces hospital parking rules and regulations; monitors the ambulance entrance and parking areas. h)Upon request, assists nursing personnel in restricting disturbed or rambunctious patients in the In-patients and/or Emergency area or where ever necessary. i)Upon request escorts hospital personnel, patients and/or visitors to and from parking areas. j)Uses daily sign-in journal, makes notation of unusual circumstances as necessary and submits a brief written report on any circumstances out of the ordinary, but always completing the daily Security Officer’s log. k)May perform other duties as assigned such as, but not limited to, emergency transportation of specimen to Tulsa Area Health Care facilities. l)Enforce all hospital policies and procedures (i.e., no smoking, or use of tobacco on hospital property). 3.MINIMUM PROFESSIONAL QUALIFICATIONS OF CONTRACTOR: Contract must have CLEET certification and at least 1 year experience or past law enforcement experience. 4.SCHEDULE AND MONITORING CONTACT: The contractor will work directly with the Department of Maintenance in the schedule. Point of contact for this contract will be Charles Jones at 918/342-6484. 5.CONTRACTOR RESPONSIBILITY: The Contractor shall obtain all necessary licenses and/or permits required to perform Security Guard services. Contractor shall obtain and be covered by Workmen’s Compensation Insurance as required under Title 85, Section 3 of Oklahoma Statues.
