- Request for Proposal, Performance Work Statement (PWS) Air Force Medical Operation Agency (AFMOA) Office of the Surgeon General. Couples Therapy for Post Traumatic Stress Disorder. 1. The Government is contemplating the issuance of a Firm Fixed Price contract. The contract is for a one year period. Anticipated period of performance is 30 Aug 2008 through 29 Aug 2009. Please review attachment 1 PWS dated 5 May 2008 and attachment 2 Training Dates dated 1 Sep 2008 and provide a complete proposal for the project. 2. The period of performance for this requirement is 12 months. We request you provide a proposal for this effort based on the attached PWS and Training Dates that address the following areas: a. Identify key personnel proposed for assignment to the project and skill level to include special qualification of key personnel. Identify persons designated as Project Manager. Submit resumes of personnel; resumes should be no longer than 2 pages. b. Comprehensive technical and management approach to accomplish the work effort. Describe knowledge and understanding of the requirement as outlined in the PWS and Training Dates. The technical approach must identify the methodology and analytical techniques to be used to fulfill the technical requirements. The technical approach should clearly decribe the following: An overview of the proposed methodology in meeting the technical requirements identified in the PWS. The logical sequence of tasks to be performed in order to accomplish the requirements. Identify and describe the specific techniques and steps that will be applied during the accomplishment of all task of this project. Any deliverables to be produced in response to project requirements. c. Describe the past performance that would qualify your firm for consideration. Describe corporate qualifications and capabilities related to the performance of the PWS requirements. Describe plan to staff and manage the service required in the PWS and include information dealing with status reporting, location of management office and proposed teaming/subcontracting arrangements to be used in support of the requirement. d. Provide a detailed cost estimate to include showing all labor categories, hourly rates and extended labor costs. Provide additional data to support any anticipated/proposed cost for Travel and Other Direct Cost. 3. The Government anticipates awarding a contract to the offeror whose proposal is the most advantageous to the Government, price and other factors considered. Award may be made to other than the lowest priced technically acceptable proposal. Proposals will be evaluated based on best value considering the following factors in order of importance: Key Personnel Qaulifications Experience/Qualification Technical Approach (clear understanding of the requirement) Past Performance and Corporate Qualifications Cost/Price Offeror price proposals will be evaluated to determine price realism and reasonableness. Prices that are excessively high or low may be considered unrealistic and unreasonable and may receive no further consideration. 4. Please e-mail Tracy Eubanks a copy of your proposal and the cost proposal spreadsheet in Microsoft Excel format by 3 PM (CST) 29 Jul 2008. Performance on the work to be negotiated under this order shall not proceed until your firm receives a written notice from the Contracting Officer. 5.
