Description
See Numbered Note 1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This is solicitation number W91YTZ-08-R-0133 and is a request for proposal (RFP). This solicitation is Unrestricted. The Standard Industrial Classification Code is 8000 and the small business size standard is $6.5M. The North American Industry Classification System Code is 621320. Fort McCoy, Wisconsin has a requirement for a Optometry Services. The resultant contract will be a base with four 1 year option periods. Overtime is not authorized. Health care providers (employee) shall not exceed $400,000.00 per year based on Title 10 U.S.C. Section 1091 and DoD Instructions 6025.5. All offers received in response to this solicitation will be evaluated. Responses are to be emailed to janet.lamb@amedd.army.mil no later than 900 hours 29 July 2008. Please be advised that all contractors doing business with the Department of Defense must be registered with CCR (the website may be accessed on the inte rnet at: www.ccr.gov to register). Offerors responding to this announcement shall provide all information contained in the Federal Acquisition Regulation (FAR) Provision 52.212-1, Instructions to Offeror-Commercial Items. As stated in this provision, the G overnment intends to evaluate offers and award a contract without discussions with offerors. Therefore, the offerors initial offer should contain the offerors best terms from a cost or price standpoint. However, the Government reserves the right to conduct discussions if later and determined by the Contracting Officer to be necessary.
