- This is a combined synopsis/solicitation from the Veterans Affairs, VA Medical Center for a used truck and equipment and is prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is VA-263-08-RQ-0326 and is issued as a Request for Quotes (RFQ). RFQ VA-263-08-RQ-0326 may be viewed thru a link to the Federal Business Opportunities @ http://www.fedbizopps.gov. No further notice will be posted on Fedbizopps. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-18. This is a Set Aside for Small Business. The NAICS is 441120. The small business size standard is $22.5 million. This RFQ will be awarded based upon best value. The statement of work for these services is as follows: Name brand GENERAL: Used 2005 Ford 550 4x4 regular cab truck chassis or approved equal. Truck to be equipped with a 9 ft Curtis Stainless Steel Spreader with separate power supply or approved equal; 9 foot Commercial Boss V-Plow adjustable, or approved equal; truck body with fold down sides and drop tail gate, hydraulic tilt box or approved equal. Automatic transmission( 5 speed, two speed transfer case) diesel engine, Tires 225/70R 19.5 or approved equal. PLACE/DELIVERY AND PERIOD OF PERFORMANCE: VAMC, 2501 W 22nd St, Sioux Falls, SD 57105. The telephone number (605) 333-6821. Delivery date is by September 4, 2008. INSPECTION AND ACCEPTANCE: At VAMC, Sioux Falls, SD and will be subject to observation and inspection by Veterans Affairs personnel. Potential vendors with vehicles equipped with noted accessories must allow inspection by the COTR and present the truck and mounted equipment offered to within a 50 mile radius from Sioux Falls, SD 57104. Inspections must occur before August 19, 2008 with the COTR prior to acceptance of bid. GOVERNMENT FURNISHED EQUIPMENT OR MATERIALS: None. All evaluation factors other than cost or price, when combined are significantly more important than cost or price. Offerors are to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer or may access a link at http://ccr.gov. Federal Acquisition clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. Federal Acquisition Regulation clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition and includes applicable clauses under the simplified threshold. This announcement constitutes the only solicitation. Ensure your RFQ includes the following: Price of truck, spreader, and plow as a fully operational unit price.
