- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Award is contingent upon availability of FY09 funds. Solicitation W45XTR82761052 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-06. This acquisition is under NAICS code 339113. C.R. Darnall Army Community Hospital (CRDAMC), Fort Hood, TX plans to award a contract for VAPR GENERATORS AND ELECTRODES in accordance with the following Salient Characteristics: SALIENT CHARACTERISTICS: The MITEK VAPR Electrosurgical System, when used with a VAPR Electrode or VAPR Flex Electrode, is intended for resection, ablation, excision of soft tissue, hemostasis of blood vessels and coagulation of soft tissues in patients requiring arthroscopic surgery of the knee, shoulder, ankle, elbow, and wrist. The MITEK VAPR Electrosurgical System, when used with a VAPR T Thermal Electrode, is intended for coagulation of soft tissues in patients requiring arthroscopic surgery of the knee, shoulder, ankle, elbow, and wrist. The requirements for this solicitation are: CLIN 0001, LEASE OF VAPR GENERATORS VAPR GENERATORS, 3 EACH, (3 VAPR GEN ALREADY ON HAND, DO NOT NEED ANY ADDITIONAL ONES) SN# 0623742, 0624348 AND 0624181. VENDOR WILL REPLACE ANY OF THE VARP3 UNITS SHOULD THEY UPGRADE AND REPLACE ALL PART TO REPAIR BROKEN UNIT AT NO COST TO THE GOVERNMENT. UNITS ARE CURRENTLY LOCATED IN THE OR AT CRDAMC BLDG 36000 2ND FLOOR, FORT HOOD TX 76544, GOV'T POC: VALARIE LAWYER-JEFFERSON @254-288-8793. CLIN 0002, SUPPLIES FOR THE VARPS SUPPLIES NEEDED FOR THE VARP3 GENERATOR: CAT# 227305: VAPR3.5MMHOOK 100 each. CAT# 225252: VAPR2.3 END ELECCTRODE 140 each. CAT# 225370: VAPR S-90 4.0 1460 each. The quantities are estimated and orders will be placed on an as needed basis. Payment will be made for actual quantities ordered and delivered to the government. All quotes shall include delivery dates expressed in days or weeks from award date. Any award resulting from this RFQ will be issued on Standard Form 1449 and will contain all clauses required by law, the Federal Acquisition Regulation (FAR), and the Defense Federal Acquisition Regulation Supplement (DFARS) as appropriate for the dollar value of the award. Provision 52.212-1, Instructions to Offerors - Commercial; Provision 52.212-2, Evaluation Commercial items, The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation provides the Best Value and will be most advantageous to the Government, price and other factors considered. All proposals will be evaluated in accordance with the following criteria: Technical Capability (compliance with the requirements of FAR Clause 52.211-6 if vendor offers an Equal Product) and Price, with Technical Capability being significantly more important than Price. Vendors shall include a completed copy of FAR provision 52.212-3 with their offer. The provision can be downloaded by accessing the website listed in this solicitation. Clause 52.212-3, Offeror Representations and Certifications - Commercial Items; Clause 52.212-4, Contract Terms and Conditions - Commercial Items; and Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items; Clause 252.232-7003 Electronic Submission of Payment Requests; Clause 252.212-7001 Contract Terms and Conditions; and Clause 252.232-7003 Army Electronic Invoicing Instructions, are applicable to this acquisition, other clauses may be included in the award if applicable. Payment will be made via Wide Area Workflow (WAWF) in accordance with DFARS 252.232-7003. FAR provision 52.212-1 [Instructions to Offerors Commercial] applies to this acquisition to include the following addenda: All offerors are cautioned that if selected for award, they must be registered with the Central Contractor Registration (CCR) program. The CCR can be accessed via the internet at www.ccr.gov/. Confirmation of CCR registration will be validated prior to awarding a contract. Evaluation Process: The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered; All proposals will be evaluated in accordance with the following EVALUATION CRITERIA: Technical Capability and Price with Technical Capability being significantly more important than Price. However, Price could become the determinative selection factor if the quality of the proposals is determined to be essentially equal, or if a proposal deemed superior in technical quality is determined not to be worth the high price premium. The trade-off between technical merit and price could result in awarding to other than low offerors. Vendors shall include a completed copy of FAR provision 52.212-3 with their offer. The provision can be downloaded by accessing the website listed in this solicitation. All quotes should also include a copy of your commercial price list or price support along with your companys Cage Code, Taxpayer Identification Number, and DUNS Number.
