- Statement of Work 1. Scope:a. The contractor is required to provide all services, materials, andequipment necessary for the repair/maintenance of handpieces located in the Branch Dental Clinic San Diego, Material Management Naval Medical Center San Diego to ensure dependable and reliable equipment operation. The scope of work performed under these specification includes the furnishing of all labor, and parts to perform all repairs on equipment listed, to assure continued operation at their designed efficiency and capacity. 2. Location:a. Branch Dental Clinic San Diego: 2310 Craven St, Bldg. 3231, SanDiego, CA 92136 3. Preventive Maintenance:a. Perform service repair/maintenance to industry standards.b. Ensure that only FULLY QUALIFIED FIELD Et"-lGINEERS ANDTECHNICIANS who have gone through original equipment manufacturer or comparable third party service schools for the above mentioned equipment, shall be employed in the performance of any and all work performed under this contract. Upon request, the contractor shall provide training certificates (or notarized copies) to the Material Management Department, Naval Medical Center San Diego for verification. The highest standard of professional capability and electrical/mechanical workmanship is to be maintained throughout the life of this contract.c. Make repairs to the extent necessary (as determined by inspection tests or disassembly) to ensure a functional system that will efficiently serve its intended purpose. 4. Corrective Maintenance:a. Correct inoperable condition in a timely manner. Immediately uponcontract award, the contractor will provide Material Management Department, Naval Medical Center San Diego, an emergency telephone number. Contractor will respond no later than 2 hours after telephone notification.b. Provide only the work necessary to restore the equipment to a serviceable/operating condition by adjustments, replacement parts, or minor repairs when it is determined that extensive repairs and parts replacements are not necessary. c. Equipment improvement/modification shall be made only upon Material Management Department, Naval Medical Center San Diego written approval and directions.d. Notify the Material Management Department, Naval Medical Center San Diego immediately upon receipt of OEM or replacement parts/equipment safety recall notices.e. Ensure that original design and functional capabilities will not be changed, modified, or altered unless the Material Management Department, Naval Medical Center San Diego authorizes such charges.f. Provide suitable OEM recommended repair equipment/toolsrequired for the satisfactory execution of all repairs made.g. Furnish manufacturer OEM approval lubricants and lubricate wearpoints within the equipment.h. Extend to the government all commercial warranties onreplacement parts, consistent with standard industry inventory. 5. Title To Equipment:a. The contractor shall not assume possession or control of any part ofthe equipment. The government retains ownership to title thereof. 6. Liabilities:a. The contractor shall not be liable for any loss, damage, or delaydue to any cause beyond his reasonable control including but not limited to, acts of government, strikes, lockouts, fire, explosion, theft, floods, riot, civil commotion, war, malicious mischief or acts of God. 7. Utilities:a. The contractor may use Government utilities (e.g., electrical power,compressed air, and water) that are available and required for any service performed under this contract. The building engineer, to ensure compatibilities with the Naval Medical Center's electrical wiring and equipment, must approve contractor electrical equipment. 8. Shipment:a. The contractor shall provide reasonable shipment for all equipmentcoming from and going to Branch Dental Clinic San Diego. 9. Parts Availabilities:a. To ensure minimal equipment downtime, the contractor shallmaintain replacement repair parts and material necessary to perform each repair or supply said parts and material within 2 days. 10. Labor:a. All compensation for labor is included in the contract price. Partsand materials: All compensation for parts and materials is included in contract price. 11. Service Report:a. The contractor, or his representative, shall furnish a legible copy ofthe service report upon completion of work performed.
