Description
The Federal Bureau of Prisons, Field Acquisition Office, Grand Prairie, Texas, intends to make a single award to a responsible quoter for the provision of Optometry Services for the Federal Medical Center and Federal Prison Camp located in Lexington, Kentucky 40511. The North American Industry Classification System (NAICS) Code is 621320 (Size Standard: $7.0 million). All duties shall be performed in accordance with standards and methods generally accepted within the field of Optometry services, and compliance with all Federal and State policy and regulations. The contractor’s performance shall be monitored by the COTR and the contractor must have the ability to work with an inmate population from a variety of cultural, regional, racial and ethnic backgrounds. The contractor shall conduct various optical examinations, treatments and provide written reports. The contractor shall be experienced as an Optometrist and be a graduate of an accredited school of Optometry. Finally, the contractor must also possess and maintain a current license and registration for the state of Kentucky. It is estimated that the contractor will perform 576 one hour sessions per year. Four one (1) hour sessions are normally conducted during each visit. It is anticipated that the contractor will at a minimum, visit the institution twice each week. The contract period will consist of a 12-month base period and four 12-month option periods for renewal. The solicitation will be available on or about December 11, 2008, and will be distributed solely through the website at www.fedbizopps.gov. All future information concerning the acquisition, including solicitation amendments will be distributed through the website. The anticipated date for receipt of quotes is December 31, 2008. The solicitation is restricted to small business sources. All responsible small business sources may submit a quotation which will be considered. Faith-Based and Community-Based Organizations can submit offers/bids/quotations equally with other organizations for contract for which they are eligible. Please note: All contractors doing business with the Federal Government must be registered in the Central Contractor Registration (CCR) database. The website for registration is www.ccr.gov.
