- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is 090001 and is issued as a Request for Quote (RFQ). The NAICS code is 621210 and the size standard is less than $7.0 MIL. The solicitation document and incorporated provisions and clauses are those in effect though Federal Acquisition Circular 2005-27, Effective 17 Oct 2008. The proposed contract is intended for a set a side for small business concerns. The acquisition is a firm fixed priced purchase order in support of the Montana Army National Guards Montana Medical Command (MMC). The MMC requires professional services for a dentist to travel to Montana to support Soldier Readiness Processing for possible deploying soldiers. Travel and per diem costs shall be included in the price of the proposal submitted to the contracting officer. If travel is not included in the price of the proposal, the proposal will be excluded from the bidding process. Contract the below POC for the Statement of Work and Annex for full details of these requirements. Dentists must be licensed, credentialed, and insured. Contractor must maintain appropriate credentialing files on employees. Contractor will supply credentials to MTARNG prior to contract date. ANY questions will be emailed to the below POC BEFORE 7 Nov 08 by 2:00 pm. NO PHONE CALLS Accepted. The provision of FAR 52.212-3, Offeror Representations and Certifications-Commercial Items (May 2004) applies to this RFQ. 52.212-3, the clause needs to be completed and a signed copy of this provision shall be submitted with any offer. 52.212-2, Evaluation, Commercial Items (Jan 1999). The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Technical Capability, Price, and Past Performance. Past performance and technical capability, when combined, are equal when compared to price or cost. Vendors are requested to submit past performance information to include persons of contact on their last two contracts for similar products.
