- The Aberdeen Area Indian Health Service is currently seeking potential quoters for a Firm Fixed-Price, Non-Personal Healthcare Service Purchase Order for a Psychiatrist to provide services three (3) days per week at the White Earth PHS, IHS Health Center in White Earth, Minnesota. The period of performance will be from Date of Award through September 30, 2009 or 432 hours; whichever comes first. The Contractor shall perform according to the Scope of Work. The unit price is an all inclusive cost which is to include travel, lodging, per diem, fringe benefits, federal, state and local taxes, plus all other costs pertinent to the performance of the purchase order. Requests for the Request for Quotations (RFQ) package, RFQ-09-012, shall be in writing either by email or fax. In the written request please reference RFQ number RFQ-09-012 and provide a postal mailing address, email address, and fax number where the RFQ package can be sent. The RFQ package will be released approximately fifteen (15) days after the date of this publication which will be November 20, 2008. The closing date of the RFQ package is December 5, 2008. This is small business set-aside.
