- The U.S. Department of Commerce, National Oceanic and Atmospheric Administration (NOAA), Office of Marine and Aviation Operations (OMAO), requires a contractor to provide medical advice, programmatic support, medical direction, and clinical oversight to nurse practitioners and physicians assistants working on the NOAA fleet of vessels. The following services are to be provided: PERSONNEL: The Contractor's Medical Director, who shall be a board certified emergency physician, will have overall responsibility for the medical services provided within the scope of the resultant contract. An assistant medical director will have overall responsibility when the medical director is not available. MEDICAL ADVICE AND CONSULTATION: Medical advice calls to contractor will be initiated by a NOAA Medical Officer on an as needed basis. The successful contractor will be required to provide a group of board certified emergency physicians to serve as on-line medical control on a 24 hours a day, seven-days a week basis to respond to requests for medical advice calls. All medical advice calls, and any subsequent follow-up calls, will be managed by the contractor's attending physician, who will communicate directly with the NOAA Medical Officer. The Attending will provide consultation, assessment, recommendation for treatment, determination of level of service and advice regarding whether an evacuation is deemed necessary to the Medical Officer, who shall be responsible for making the final decision regarding patient care. The contractor will maintain a separate phone-line designated for contract related emergency calls, with back-up phone numbers available for Remote Client. The contractor will provide NOAA with a communication manual for each vessel covered by the contract containing information about accessing the Contractor's services and a set of medical assessment guidelines. A client report of each medical advice case will be sent to the Director, Office of Health Services, NOAA, upon initiation of the case. In addition to the requirements stated above, the contractor will be required to provide medical consultation by a board certified emergency physician for the following services: a). Not Fit for Duty: Written medical review of any physical examination identified as Not Fit for Duty by NOAA Health Services personnel. b). Medical Consultation: Physician review of medical concerns raised by NOAA Health Services including policy, test results, physical exam findings, health issues potentially impacting other NOAA employees, other medical concerns as identified by NOAA Health Services. The Contractor will communicate information on the Remote Client employee to Director of Health Services of the Remote Client within twenty-four hours, and on an as needed basis, consistent with all applicable law regarding such disclosures, until the case is closed. A final written report will be sent to Director of Health Services of Remote Client upon closure of the case consistent with applicable law regarding such disclosures. MEDICAL DIRECTION/CLINICAL OVERSIGHT: The contractor will provide physician medical direction and clinical oversight for the six Physician Assistants (P.A.'s) and four Nurse Practitioners (N.P.'s) practicing with NOAA. The following services will be included: a) Clinical supervision provided by the Contractor's Medical Director and Assistant Medical Director. b) Review of current practice guidelines for on site medical practitioners. c) A quarterly review of: 1)A minimum of 5% patient care reports from the P.A. onboard the vessels. 2)Every medical case where controlled substances were prescribed, whether by a P.A. or N.P. 3)Medical patient encounter logs generated by P.A.s and N.P.s to include crewmember's name and chief complaint. The current NAICS code is 621111 with a size standard $9 million. The estimated date of release of this commercial item solicitation is November 27, 2008 with a planned closing date of December 11, 2008.
