- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation number VA-244-09-RQ-0047 is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-29. The North American Industry Classification System (NAICS) number is 339112 and the business size standard is 500 employees. This requirement is 100% set aside for small businesses. This is a brand name or equal acquisition, 52.211-6 applies. Vendors must supply installation and warranty on parts, labor and travel and two copies each of service and operating manuals as part of your quote. Delivery shall be FOB Destination, VA Medical Center, Wilkes-Barre. REQUESTERS MUST STATE THEIR BUSINESS SIZE ON EACH QUOTATION. The Department of Veterans Affairs Medical Center, 1111 East End Blvd, Wilkes-Barre, PA 18711, is accepting quotations for the following line items: (1) One BeaconMaedes LLDA-105D-RKC Desiccant Dryer Duplex System or equal, which must include: Twin tower desiccant dryers, stainless steel separators, stainless steel pre-filters, after-filters, dew point hygrometer and internal purge saving control system within a four-valve bypass arrangement. Fully compliant with the latest edition of National Fire Protection Association (NFPA) Standard 99. (2) Installation (must be coordinated with VA Medical Center Staff) to be completed during the hours of 8:00AM and 8:00PM. (3) Remove and set aside current air dryers for VA disposal to be completed during the hours of 8:00AM and 8:00PM. (4) On-site training. Also include in price - twenty four (24) months warranty in materials and workmanship from date of start-up. Also must include two copies of service and operating manuals. Delivery is required within eight weeks after receipt of order. The VAMC has determined such product is the only item that can meet its needs. Offerors must comply with all instructions contained in FAR 52.212-1, Instructions to Offerors-Commercial Items. FAR 52.212-1 is supplemented per the following addenda: Contractor shall submit their quote on SF 1449, provide solicitation number, the time specified in the solicitation for the receipt of offers, name, address and telephone number of the offeror, unit price, and the overall price. A technical description (include information illustrating that your product meets our technical requirements), any discount terms, CAGE Code, DUNS number The following provisions and clauses apply: The offeror must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items. Clause 52.212-4, Contract Terms and Conditions-Commercial Items applies to this acquisition with the following provision/clauses added by Addenda: 852.270-4, Commercial Advertising. In addition, Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition and the following clauses as listed under 52.212-5 apply: 52.219-6, Notice of Total Small Business Set-Aside; 52.219-28, Post Award Small Business Program Re-representation; 52.222-19, Child Labor - Cooperation with Authorities and Remedies; 52.222-21, Prohibition of Segregated Facilities; 52-222-26, Equal Opportunity; 52.222-36, Affirmative Action for Workers with Disabilities; 52.222-50, Combating Trafficking in Persons; 52.225-3, Buy American Act - Free Trade Agreements - Israeli Trade Act; 52.225-13, Restrictions on Foreign Purchases; 52.232-34, Payment by Electronic Funds Transfer - Other than Central Contractor Registration, 52.239-1, Privacy or Security Safeguards. This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text. Upon request, the contracting officer will make their full text available. VA will base its award on lowest price, most technically acceptable offer. Award will be made on an all or none basis; whereas the awardee must be capable of providing all line items to be eligible for award. Your company must be actively registered in CCR to be considered for award. Quotations must include the RFQ number, the proposed unit price, the proposed extended price, the proposed delivery schedule, any prompt payment discount terms, company name, and point of contact, address, phone number, fax number, tax identification number, DUNS number, and size status (i.e., disabled-veteran-owned business, veteran owned business, disadvantaged business or woman owned business or small business).
