- The Treasure Island Job Corps is a federally funded Vocational & Educational Training Program for youth between the ages of 16 and 24. The program operates year round by ResCare for the United States Department of Labor. The Treasure Island Job Corps has approximately 600 residential and non-residential trainees. The Treasure Island Job Corps is soliciting bids for the following on-site Medical Services: 1. Medical Physician to work on our site for a minimum of 24 hours* using our facilities, equipment, and supplies. Must have graduated from an accredited medical school and have a valid physician license to practice in the State of California. Provide medical services for trainees ages 16 thru 24. Includes exams, diagnosis and treatment of illness and injury, detection and control of communicable diseases, records keeping, effective referrals for special care and participating in medical health education programs 2. Certified Nurse Practitioner - Complete physical examination on adolescent or young adult on on-site clinic, for a minimum of 24 per week* Must have graduated from an accredited medical school and have a valid license to practice in the State of California. *Minimum work hours indicated for each of the above services are subject to change upon the discretion of the Center Director. The terms of the contract will be for one year commencing on February 1, 2009 and continuing until January 31, 2010 with the option to extend this contract's period for up to four years in successive one year increments.This option is at the Center’s discretion to exercise. Bids must indicate rate per hour. Bids must be submitted using the below format: Contract period 2/1/09 to1/31/10 Hourly_____________ Contract period 2/1/10 to1/31/11 Hourly_____________ Contract period 2/1/11 to1/31/12 Hourly_____________ Contract period 2/1/12 to1/31/13 Hourly_____________ Contract period 2/1/13 to1/31/14 Hourly_____________ Bids must be received by December 29, 2008 via fax or email to the contact person below. Successful bidder must comply with all Federal Acquisition Regulation clauses and be compliant under all related federal statutory requirements and regulations. Additionally, applicants must submit a Resume or Curriculum Vitae and a certificate of liability/malpractice insurance. An appointment to view the facility must be requested from the Purchasing Supervisor via email or fax. No site tour will be entertained without prior appointment.
