- The Alabama National Guard, Office of the State Surgeon Alabama, located at 5401 East Lake Blvd, Bldg 550, Birmingham, Alabama and the USPFO for Alabama, 1740 Cong. W. L. Dickinson Drive intends to issue a Request for Proposal (RFP) to award a single, Indefinite Delivery Indefinite Quantity (IDIQ) contract for services, non-personal, to provide all labor, transportation, materials, facilities, tools, equipment, appliances and supervision necessary to perform and complete tasks that cover the various components of Individual Medial Readiness (IMR) and Fully Medically Ready Medical and Dental health force-sustainment for Army National Guard (ARNG) forces. Components are to include but not limited to: Medical: Blood Pressure, height & weight, and questionnaire screening, Periodic Health Assessments (PHAs), Specialty Physical Exams such as Chapter 2 (schools), 'Over-40,' Flight, etc. in accordance with AR (Army Regulations) 40-501, laboratory / phlebotomy work such as HIV, urine pregnancy, pelvic exams (pap-smears), DNA, G6PD,RPR, Immunizations required for IMR/FMR and theater specific compliance such as Hepatitis A/B, Influenza, PPD (in some cases, vacancies to be supplied by ARNG), visual acuity and Optometry exams, DOEHRS compatible audiometry exams and SPRINT testing, MEDPROS entry of all applicable and appropriate IMR/FMR functions, and Post Deployment Health Re-Assessments (PDHRA). Dental; annual dental exams, digital bitewings radiographs, digital panoramic radiographs, DENCLASS data entry of all radiographs and dental records, identification and treatment (to 90+%) of all Class 3 soldiers on alert orders and eligible Services are to be completed on site at various ARNG Armories & Training Sites across the State. Period of performance for Indefinite Delivery Indefinite Quantity (IDIQ) award is one (1) year and may be, at the governments option, extended for four option years. The North American Industry Classification System (NAICS) codes for this work is 621399 - Offices of All Other Miscellaneous Health Practitioners - small business size standard is $7,000,000 average annual revenue for the previous three years and 621999 - All Other Miscellaneous Ambulatory Health Care Services - small business size standard is $10,000,000 average annual revenue for the previous three years. This action is being procured on a 100% Small Business Set Aside basis in accordance with the Small Business Competitiveness Demonstration Program (Ref: FAR 19.10). The Price evaluation preference for HUBZone small business concerns in accordance with FAR 19.1307 applies. The tentative date for issuing the solicitation is on-or about 29 January 2010. Anticipated award date is 15 March 2010. Interested offertory parties must be registered in the DOD Central Contractor Registration (CCR) database to be considered for this award. Registration can be accomplished at https://www.bpn.gov/ccr/default.aspx Representations and Certifications (Reps and Certs) process will be accomplished within the Online Representations and Certifications Application (ORCA) web portal found at https://orca.bpn.gov/ This solicitation is a competitive Request for Proposal and a formal source evaluation and selection will be done. Amendments to solicitation, if/when issued will be posted to the Federal Business Opportunities, https://www.fbo.gov/ for electronic downloading.
